摘  要:探讨乙酰谷酰胺注射液联合纳洛酮对急性重症酒精中毒复苏疗效及抗氧化功能的机制,选择2017年5月至2019年5月该院接诊的95例急性重症酒精中毒患者,通过随机数表法分为研究组50例和对照组45例,两组均接受常规处理,对照组在此基础上,给予盐酸纳洛酮注射液治疗,研究组在对照组基础上给予乙酰谷酰胺注射液治疗。比较两组治疗起效时间、显效时间和痊愈时间,并观察治疗前、治疗12 h后抗氧化功能指标、神经递质指标、肝功能指标的变化及安全性。研究组的治疗起效时间、显效时间、痊愈时间分别为(70.86±14.14) min、(165.64±33.69) min、(245.36±33.13) min,对照组分别为(96.24±17.95) min、(231.84±38.18) min、(313.89±34.79) min,差异均有统计学意义(P <0.05);治疗后12 h,抗氧化功能指标中,研究组总抗氧化能力(TAC)、谷胱甘肽过氧化物酶(GSH-Px)、超氧化物歧化酶(SOD)、丙二醛(MDA)分别为(12.34±1.66) k U/L、(138.37±16.64) U/L、(94.36±11.65) U/mL、(5.57±0.62)μmol/L,对照组分别为(10.16±1.45) kU/L、(124.32±13.73) U/L、(78.54±8.08) U/mL、(8.12±0.97)μmol/L,神经递质指标中,研究组多巴胺(DA)、5-羟色胺(5-HT)分别为(145.69±13.31) ng/L、(141.03±14.97) ng/mL,对照组分别为(164.57±14.54) ng/L、(171.32±13.73) ng/mL,肝功能指标中,研究组谷草转氨酶(AST)、谷丙转氨酶(ALT)、γ-谷氨酰转肽酶(γ-GT)分别为(32.32±1.56) U/L、(29.74±1.84) U/L、(24.19±1.82) U/L,对照组分别为(34.21±1.63) U/L、(34.23±2.31) U/L、(27.56±1.78) U/L,差异均有统计学意义(P <0.05);两组治疗后均未出现严重酒后并发症及药物不良反应,无死亡病例。乙酰谷酰胺注射液联合纳洛酮治疗急性重症酒精中毒患者的复苏效果显著,和单独使用纳洛酮相比,联合治疗可进一步改善机体抗氧化能力,促进神经递质、肝功能的恢复。To study the resuscitation effect of aceglutamide injection combined with naloxone in treatment of acute severe alcoholism and its mechanism of antioxidant function,95 patients of acute severe alcoholismand who received therapy from May 2017 to May2019 in the author’s hospital were selected,based the random number table were divided into 50 cases in the research group and45 cases in the control group,they received routine treatment and on this basis,the control group was given naloxone hydrochloride injection,the research group was combined with aceglutamide injection on the basis of the control group. The onset time,effective time and recovery time of the two groups were compared,and the changes of antioxidant function index,neurotransmitter index and liver function index before and 12 hours after treatment and safety were observed. The onset time,effective time and recovery time in the research group were( 70. 86 ± 14. 14) min、( 165. 64 ± 33. 69) min、( 245. 36 ± 33. 13) min respectively,in the control group were( 96. 24 ± 17. 95) min、( 231. 84 ± 38. 18) min、( 313. 89 ± 34. 79) min respectively,the differences were statistically significant( P < 0. 05). After treatment 12 hours,the total antioxidant capacity( TAC),glutathione peroxidase( GSH-Px),superoxide dismutase( SOD),malondialdehyde( MDA) in the research group were( 12. 34 ±1. 66) k U/L、( 138. 37 ± 16. 64) U/L、( 94. 36 ± 11. 65) U/m L、( 5. 57 ±0. 62) μmol/L respectively,in the control group were( 10. 16 ± 1. 45) k U/L、( 124. 32 ± 13. 73) U/L、( 78. 54 ± 8. 08) U/m L、( 8. 12 ± 0. 97) μmol/L respectively,in the neurotransmitter indexes,the dopamine( DA) and serotonin( 5-HT) in the research group were( 145. 69 ± 13. 31) ng/L、( 141. 03 ± 14. 97) ng/m L respectively,in the control group were( 164. 57 ± 14. 54) ng/L、( 171. 32 ± 13. 73) ng/m L respectively,in the liver function index,the glutamic pyruvic transaminase( AST),alanine aminotransferase( ALT) and γ-glutamyl transpeptidase( γ-GT) in the research group were(
