奥曲肽注射液治疗食管-胃底静脉曲张破裂出血的有效性研究

摘　　要：目的:探讨奥曲肽注射液治疗食管-胃底静脉曲张破裂出血的临床效果。方法:2018年2月-2019年6月收治食管-胃底静脉曲张破裂出血患者116例,根据随机数字表法分为两组。对照组给予醋酸去氨加压素注射治疗;观察组在醋酸去氨加压素基础上运用奥曲肽注射液治疗。比较两组治疗效果。结果:观察组治疗后肝动脉血流量和门静脉血流量均低于对照组,差异有统计学意义(P<0.05);观察组血压恢复正常时间、心悸缓解时间、黑便消失时间及呕血消失时间均低于对照组,差异有统计学意义(P<0.05);观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05);试验组直接胆红素(DBIL)、总胆红素(TBIL)、谷草转氨酶(AST)及谷丙转氨酶(ALT)水平均低于对照组,差异有统计学意义(P<0.05)。结论:临床运用奥曲肽注射液治疗食管-胃底静脉曲张破裂出血,不仅可以减轻患者症状,还能提高治疗效果。Objective:To explore the clinical effect of octreotide injection in the treatment of esophagogastric variceal hemorrhage.Methods:From February 2018 to June 2019,116 patients with esophageal and gastric variceal vein rupture and hemorrhage were selected to our hospital and randomly divided into two groups.The control group was treated with desmopressin acetate injection.Observation group was treated with octreotide injection on the basis of desmopressin acetate.The therapeutic effects between the two groups was compared.Results:The hepatic arterial blood flow and portal venous blood flow in the observation group were lower than the control group,with the differences were statistically significant(P<0.05).The time of blood pressure return to normal,palpitation relief time,melena disappearance time and hematemesis disappearance time in the observation group were lower than the control group,with the differences were statistically significant(P<0.05).The total effective rate of the observation group was higher than the control group,with the difference was statistically significant(P<0.05).The levels of direct bilirubin(DBIL),total bilirubin(TBIL),aspartate aminotransferase(AST)and alanine aminotransferase(ALT)in experimental group were lower than the control group,with the differences were statistically significant(P<0.05).Conclusion:Clinical application of octreotide injection in the treatment of esophageal-gastric variceal hemorrhage can not only alleviate the symptoms of patients,but also improve the therapeutic effect.
