标准化评估与综合干预在完全植入式输液港日常维护中的效果评价

摘　　要：目的探讨和评价标准化评估与综合干预在恶性肿瘤患者完全植入式输液港(TIVAP)日常维护中的效果。方法选取本院肿瘤科2018年1月至2020年2月接受完全植入式输液港患者85例,按照数字随机表均分为对照组(n=42)和观察组(n=43)。对照组实施TIVAP常规护理及日常维护,观察组实施全程标准化评估与综合干预管理模式。干预8周,对比两组TIVAP留置时间、相关并发症、随访半年的负性情绪(SAS/SDS)及生活质量(GQOL-74)结果。结果干预8周后,观察组TIVAP平均留置时间明显长于对照组(P<0.05);观察组TIVAP相关并发症发生率明显低于对照组(P<0.05);干预后随访半年,观察组SAS/SDS评分均明显低于对照组(P<0.05);观察组生活质量各维度得分及总分均显著高于对照组(P<0.05)。结论标准化评估与综合干预能明显改善完全TIVAP留置患者日常维护效果,延长留置时间,降低并发症发生率,患者不良情绪和生活质量得到明显改善。Objective:To explore and evaluate the effect of standardized assessment and comprehensive intervention in the routine maintenance of fully implantable transfusion port(TIVAP)for patients with malignant tumors.Method:Eighty-five patients who received a fully implantable infusion port from January 2018 to February 2020 in our hospital's oncology department were selected and divided into a control group(n=42)and an observation group(n=43)according to the random table.The control group implements TIVAP routine care and daily maintenance,and the observation group implements a standardized assessment and comprehensive intervention management model.After 8 weeks of intervention,the TIVAP retention time,related complications,and negative emotion(SAS/SDS)and quality of life(GQOL-74)results of six months of follow-up were compared between the two groups.Results:After 8 weeks of intervention,the average retention time of TIVAP in the observation group was significantly longer than that of the control group(P<0.05);the incidence of TIVAP-related complications in the observation group was significantly lower than that in the control group(P<0.05);follow-up for half a year after the intervention,the SAS/SDS scores in the observation group were significantly lower the control group(P<0.05);the observation group quality of life scores and total scores were significantly higher than the control group(P<0.05).Conclusion:Standardized evaluation and comprehensive intervention can significantly improve the daily maintenance effect of complete TIVAP indwelling patients,prolong the indwelling time,reduce the incidence of complications,and significantly improve the patients'bad mood and quality of life.
