摘　　要：目的探究腹腔镜下胃癌D2根治术对进展期胃癌患者术后康复及预后的影响。方法回顾性分析2017年8月至2019年8月本院收治的86例进展期胃癌患者的临床资料,依据手术方式的不同分为观察组(n=45)和对照组(n=41),观察组接受腹腔镜下胃癌D2根治术治疗,对照组接受开腹胃癌D2根治术治疗。比较两组围术期指标、手术效果及并发症发生率。结果观察组术中出血量少于对照组,手术时间长于对照组,住院时间短于对照组,并发症发生率低于对照组,差异有统计学意义(P<0.05);两组肿瘤距近切距离、距远切距离及淋巴结清扫数目比较差异无统计学意义。结论进展期胃癌患者接受腹腔镜下胃癌D2根治术治疗安全可行,手术效果好且创伤小,利于减少并发症并缩短住院时间。Objective To explore the effect of laparoscopic D2 radical resection of gastric cancer on postoperative rehabilitation and prognosis of patients with advanced gastric cancer.Methods The clinical data of 86 patients with advanced gastric cancer treated in our hospital from August 2017 to August 2019 were retrospectively analyzed.They were divided into observation group(n=45)and control group(n=41)according to differ-ent surgical methods.The observation group received laparoscopic D2 radical resection of gastric cancer,the control group received open D2 radical resection of gastric cancer.The perioperative indicators,surgical effects and complications were compared between the two groups.Results The in-traoperative blood loss of the observation group was less than that of the control group,the operation time was longer than that of the control group,the hospital stay was shorter than that of the control group,and the incidence of complications was lower than that of the control group,the difference was statistically significant(P<0.05);there was no statistically significant difference in the incision distance,the distance from the remote incision and the number of lymph node dissection.Conclusion It is safe and feasible for patients with advanced gastric cancer to receive laparoscopic D2 radical resection of gastric cancer.The surgical effect is good and the trauma is small,which is beneficial to reduce complications and shorten the length of hospital stay.
