摘　　要：目的探讨脂蛋白相关磷脂酶A2(Lp-PLA2)、血清可溶性CD40配体(sCD40L)在急性脑梗死中的表达及与颈动脉斑块的关系。方法我院收治的78例急性脑梗死患者为观察组(稳定斑块32例,不稳定斑块46例),选取同期本院体检的健康人群70例为对照组,对比两组sCD40L及Lp-PLA2水平。分析不同临床特征患者颈动脉斑块情况,分析sCD40L、Lp-PLA2及二者联合对急性脑梗死患者颈动脉斑块稳定情况的预测价值。结果观察组sCD40L、Lp-PLA2水平高于对照组(P<0.05);稳定颈动脉斑块与不稳定颈动脉斑块患者年龄、合并高血压、总胆固醇、甘油三酯、吸烟、管腔狭窄程度、sCD40L、Lp-PLA2比较,差异有统计学意义(P<0.05);sCD40L及Lp-PLA2联合检测的ROC曲线下面积(AUC)最大。结论急性脑梗死患者加强监测血清sCD40L、Lp-PLA2水平对其颈动脉斑块稳定情况有较大预测价值,二者可作为评估脑血管疾病发生风险的重要指标。Objective To investigate the expression of lipoprotein-associated phospholipase A2(Lp-PLA2)and Cluster of Differentiation 40 Ligand(sCD40L)in acute cerebral infarction and its relationship with carotid plaque.Methods Seventy-eight patients with acute cerebral infarction admitted to our hospital were selected as anexperiment groupconsisting of32 patients with stable plaques and 46 with unstable plaques.Seventy healthy people who underwent physical examination in our hospital during the same period were selected as a control group.The levels of sCD40L and Lp-PLA2 between the two groups were compared.Carotid plaques in patients with different clinical characteristics were analyzed,and the predictive value of sCD40L,Lp-PLA2 and the combination of them for the stability of carotid plaque in patients with acute cerebral infarction were evaluated.Results The levels of sCD40L and Lp-PLA2 in the observation group were significantly higher than those in the control group(P<0.05).There were significant differences in age,combined hypertension,total cholesterol,triglycerides,smoking,degree of lumen stenosis,sCD40L and Lp-PLA2 between the two groups(P<0.05).Area under ROC curve(AUC)of the combination of sCD40L and Lp-PLA2 was the largest.Conclusion Monitoring serum sCD40L and Lp-PLA2 levels is significant forpredictingthe stability of carotid plaques in patients with acute cerebral infarction.Both indexes can be used as important indicators for assessing the risk of cerebrovascular diseases.
