摘  要:目的探讨宫颈癌患者血清甲胎蛋白(AFP)、肿瘤生长因子(TSGF)、胸苷激酶(TK1)水平变化情况及临床意义。方法选取宫颈癌患者115例,并以同期体检的宫颈良性疾病患者50例作为对照组。比较两组血清AFP、TSGF、TK1水平,采用ROC曲线分析血清AFP、TSGF、TK1水平对宫颈癌的鉴别价值,并比较各血清指标及联合检测对宫颈癌的诊断效能;比较不同血清AFP、TSGF、TK1水平患者的临床病理指标,分析影响患者血清AFP、TSGF、TK1水平升高的危险因素。结果宫颈癌组血清AFP、TSGF、TK1水平高于对照组(P<0.05);血清AFP、TSGF、TK1水平鉴别宫颈癌的AUC均大于0.8,血清AFP、TSGF、TK1水平鉴别宫颈癌与宫颈良性疾病的灵敏度、特异度对比无明显差异(P>0.05);AFP高水平组在分化程度为高度、已出现淋巴结转移、已出现深肌层浸润、TNM分期为Ⅲ、Ⅳ期的例数比例高于AFP低水平组(P<0.05);TSGF高水平组在分化程度为高度、肿瘤直径≥4 cm、已出现深肌层浸润、TNM分期为Ⅲ、Ⅳ期的例数比例高于TSGF低水平组;TK1高水平组在分化程度为高度、肿瘤直径≥4 cm、已出现深肌层浸润、TNM分期为Ⅲ、Ⅳ期的例数比例高于TK1低水平组(P<0.05);分化程度为高度、深肌层浸润是影响宫颈癌患者血清AFP水平升高的危险因素(P<0.05);分化程度为高度、肿瘤直径≥4 cm、深肌层浸润是影响患者血清TSGF水平升高的危险因素(P<0.05);分化程度为高度、深肌层浸润是影响患者血清TK1水平升高的危险因素(P<0.05)。结论宫颈癌患者血清AFP、TSGF及TK1水平高于宫颈良性疾病患者,其血清AFP、TSGF、TK1水平异常与分化程度、深肌层浸润等有关,且血清AFP、TSGF、TK1水平对宫颈癌的鉴别价值较高。Objective To explore the changes and clinical significance of serum alpha-fetoprotein(AFP),tumor specific growth factor(TSGF)and thymidine kinase(TK1)levels in cervical cancer patients.Methods A total of 115 patients with cervical cancer were enrolled,other 50 patients with benign cervical diseases undergoing physical examination during the same period were enrolled as the control group.The levels of serum AFP,TSGF and TK1 were compared between the 2 groups.And their differential value in cervical cancer was analyzed by ROC curves.The diagnostic efficiency of each serum index and their combination for cervical cancer was compared.The clinicopathological indexes among patients with different levels of serum AFP,TSGF and TK1 were compared.The risk factors that affected the increase of serum AFP,TSGF and TK1 levels were analyzed.Results The levels of serum AFP,TSGF and TK1 in cervical cancer group were higher than those in the control group(P<0.05).AUC values of serum AFP,TSGF and TK1 for identifying cervical cancer were all greater than 0.8.There were no significant differences in the sensitivity and specificity among serum AFP,TSGF and TK1 for identifying cervical cancer and benign cervical diseases(P>0.05).The number of cases with high differentiation,lymph node metastasis,deep muscle infiltration,and TNM staging at stage Ⅲ and Ⅳ in high-level AFP group were more than that in low-level AFP group(P<0.05).The number of cases with high differentiation,tumor diameter not shorter than 4cm,deep muscle infiltration and TNM staging at stage Ⅲ and Ⅳ in TSGF high-level group was more than that in TSGF low-level group.The number of cases with high differentiation,tumor diameter not shorter than 4cm,deep muscle infiltration and TNM staging at stage Ⅲ and Ⅳ in TK1 high-level group was more than that in TK1 low-level group(P<0.05).High differentiation and deep muscle infiltration were risk factors that affected the increase of serum AFP level(P<0.05).High differentiation,tumor diameter not shorter than 4cm and deep
