摘　　要：目的探究乙型肝炎病毒(hepatitis B virus,HBV)感染相关性肝癌(hepatocellular carcinoma,HCC)中乏氧因子(HIF-1α)、小眼畸形转录相关因子(MITF)、C反应蛋白(CRP)及中性粒细胞与淋巴细胞比值(NLR)的表达情况及其意义。方法随机收集97例初治原发性肝细胞性肝癌患者作为研究对象,按照是否感染HBV分为感染组62例和未感染组35例。采用免疫组化法检测肝癌组织中HIF-1α、MITF的表达情况,同时收集并计算出患者手术前后血清中CRP及NLR比值,并进行2组之间的比较。结果肝癌组织中均未见MITF的表达,而HIF-1α在肝癌组织中高表达。HBV感染组中HIF-1α的表达高于未感染组,差异具有统计学意义(χ^(2)=20.7,P<0.05);感染组CRP表达、NLR均高于未感染组,但差异无统计学意义(P>0.05)。结论HBV感染相关性HCC与肿瘤细胞乏氧有关,但与MITF的调节无相关关系,且与机体中CRP和NLR水平无明显相关性。Objective To investigate the expression and significance of hypoxia factor-1α(HIF-1α),microphthalmia-associcated transcription factor(MITF),C-reactive protein(CRP)and neutrophil lymphocyte ratio(NLR)in hepatocellular carcinoma(HCC)associated with hepatitis B virus(HBV)infection.Methods 97 inpatients with pathologically proven HCCs who were erolled into the study before any therapy,were randomly divided into the observation group(62 cases)and the control group(35 cases)according to whether they were infected with HBV.The expression of HIF-1α and MITF in the 2 groups were detected by immunohistochemistry,and the differences of CRP and NLR ratio in the 2 groups before operation were compared.Results There was no expression of MITF in HCC;HIF-1α was highly expressed in HCC,and the expression in HBV infection related group was higher than that in non HBV infection group,with statistical significance(χ^(2)=20.7,P<0.05);NLR in the observation group was(3.03±1.84),while that in the control group was(2.42±1.83),with no statistical significance(P>0.05).Conclusion HCC associated with HBV infection is related to hypoxia of tumor cells,but not to the regulation of MITF,and has no significant correlation with CRP and NLR levels.
