摘  要:目的探讨非放大窄带光成像内镜(NM-NBI)在早期食管癌(EC)诊断中的应用价值。方法选择早期高度疑似EC患者83例作为研究对象,所有患者均接受碘染色及非放大窄带光成像检查,以病理诊断结果为“金标准”,分析NM-NBI、碘染色及两者联合对早期EC患者的诊断效能。结果83例患者经碘染色检查,碘染色诊断早期EC与金标准一致性为0.666,一致性一般;83例患者经NM-NBI检查,NM-NBI诊断早期EC与金标准一致性为0.657,一致性一般;两种诊断方法敏感度、准确度对比,差异无统计学意义(P>0.05);NM-NBI的特异度高于碘染色,差异有统计学意义(P<0.05)。结论NM-NBI应用于早期EC患者的诊断中具有较高诊断效能,但临床仍可结合碘染色等检查方法以提高特异度,以为临床制定诊疗方案提供重要参考依据。Objective To investigate the application value of non magnifying narrow-band imaging endoscopy(NM-NBI)in the diagnosis of early esophageal carcinoma(EC).Methods 83 patients with early highly suspected EC were selected as the research subjects,all patients underwent iodine staining and non amplified narrow-band imaging examination,the diagnostic efficacy of NM-NBI,iodine staining and their combination in early EC patients was analyzed.Results 83 patients were examined by iodine staining.The consistency of iodine staining in the diagnosis of early EC and gold standard was 0.666,the consistency was general;83 patients were examined by NM-NBI,the consistency between EC in the early stage of NM-NBI diagnosis and gold standard was 0.657,and the consistency was general;compared the sensitivity,accuracy of the two diagnostic methods,there was no statistical significant difference(P>0.05);the specificity of NM-NBI was higher than that of iodine staining,the difference was significant statistically(P<0.05).Conclusion NM-NBI has high diagnostic efficiency in the diagnosis of early EC patients,but the methods of examination such as iodine staining can still be combined with to improve the specificity and provide an important reference for clinical diagnosis and treatment.
