摘　　要：目的:研究高强度超声聚焦超声(HIFU)治疗子宫肌瘤疗效及对患者雌激素受体(ER)、B淋巴细胞瘤-2基因(Bcl-2)蛋白的影响。方法:选取2016年12月—2018年12月本院收治的子宫肌瘤患者100例,根据治疗方法的不同分别行腹腔镜子宫肌瘤切除术(腹腔镜组)或HIFU治疗(HIFU组),分析两组治疗效果。结果:HIFU组恢复自主活动时间(0.6±0.2d)、住院时间(4.5±1.1d)、出血量(0)均少于腹腔镜组(P<0.05)。术后抗穆氏管激素、卵泡刺激素、黄体生成素、雌二醇水平两组无差异(P>0.05),ER、Bcl-2阳性表达两组均低于术前(P<0.05)但组间无差异(P>0.05);并发症率HIFU组(36.0%)低于对照组(56.0%)(P<0.05),术后1年内复发率两组(2.0%、6.0%)无差异(P>0.05)。结论:HIFU治疗子宫肌瘤对卵巢功能影响小,具有创伤小、恢复快、安全性高等特点。Objective: To study the effect of high-intensity ultrasound focused ultrasound(HIFU) for treating women with uterine myoma, and to analyze its influence on their levels of estrogen receptor(ER) and Bcl-2 gene(Bcl-2). Methods: From December 2016 to December 2018, 100 women with uterine myoma were included and divided into two groups based on different treatments. The women in the control group were treated with laparoscopic hysteromyomectomy, and the women in the observation group were given HIFU treatment. The clinical effect of the women in the two groups after treatment was analyzed. Results: The recovery time of voluntary activity(0.6±0.2 d), hospitalization time(4.5±1.1 d), and blood loss(0 ml) of the women in the observation group were significant lower than those of the women in the control group(P<0.05). There were no significant differences in the levels of anti-Mushi tube hormone(AMH), follicle stimulating hormone(FSH), luteinizing hormone(LH), and estradiol(E2) of the women between the two groups(P>0.05). The positive expression of ER and Bcl-2 of the women had decreased significantly after operation(P<0.05), but which had no significant differences of the women between the two groups(P>0.05). The complication rate of the women in the observation group(36.0%) was significant lower than that(56.0%) of the women in the control group(P<0.05). Conclusion: HIFU used for treating women with uterine myoma has little influence on their ovarian function with the characteristics of less trauma, rapid recovery, and high safety.
