摘　　要：目的探讨多发性硬化(MS)脊髓病灶影像学特征及其在MS不同残疾组间的差异。方法回顾性分析93例MS患者脊髓病灶MRI征象,评估以脊髓症状为主的MS患者残疾状态及分析不同残疾组的MS患者脊髓病灶特征。结果严重残疾MS患者中,脊髓内病灶数量≥3(P=0.000)、脊髓中央区受累(P=0.011)的发生率高;与轻度残疾组、中度残疾组比较,脊髓肿胀、中央管扩张、亮点征及空洞在严重残疾组更常见,但结果无统计学差异。结论不同残疾状态的MS患者脊髓内病灶特征存在差异,可根据脊髓病灶特征差异评估以脊髓症状为主的MS患者临床严重程度。Objective To investigate the imaging characteristics of spinal cord lesions in patients with multiple sclerosis(MS) and their differences among different disability groups in MS. Methods The MRI signs of spinal cord lesions in MS patients were retrospectively analyzed, the disability status of MS patients with spinal cord symptoms were evaluated, and the characteristics of spinal cord lesions in MS patients in different disability groups were analyzed. Results In severely disabled MS patients,the number of lesions in the spinal cord was ≥ 3( P = 0. 000) and the central area of the spinal cord was involved( P = 0. 011). Compared with the mildly disabled group and the moderately disabled group,the incidence of spinal cord swelling,central canal expansion,bright spot signs and voids were more common in the severely disabled group,but the results were not statistically different. Conclusion There are differences in the characteristics of spinal cord lesions in MS patients with different disability states. The clinical severity of MS patients with spinal cord symptoms can be assessed based on the differences in the characteristics of spinal cord lesions.
