基于“燥者濡之”论治慢性萎缩性胃炎胃阴虚证

摘　　要：慢性萎缩性胃炎属于消化系统常见病,多发病,日久可伤及胃阴。从燥而言,是谓胃内湿热较甚,使胃腑郁闭,湿致气和津液不得宣行布达,因而形成胃之燥象,燥象贯穿于阴虚证的全过程,故曰“阴虚者必燥”。“燥者濡之”语出《素问·至真要大论》,即以濡润之品治疗津液亏乏之症,凡能使胃恢复柔润之性的药物即可称为润剂。全国名中医刘启泉教授依据多年临床经验,基于“燥者濡之”的理论治疗萎缩性胃炎胃阴虚证。慢性萎缩性胃炎初期因气滞或者湿热伤阴,应用甘寒理气药濡养胃阴,中期由于母病及子影响至肺,宜用轻灵之药润肺胃之阴,后期依据“五脏相通,移皆有次”“五脏之伤,穷必及肾”等理论,运用咸寒补益药资胃肾之阴,以恢复胃阴濡润及升降之性。Chronic atrophic gastritis(CAG)is a common and frequently-occurring disease in digestive system,which can injure stomach yin over time.As far as dryness is concerned,it means that the damp heat in the stomach is rather severe,which makes the stomach shut up,and dampness causes qi and body fluid to be prevented from spreading,thus forming the dryness of the stomach,which runs through the whole process of yin deficiency syndrome,so it is said that“yin deficiency must be dry”.The phrase“dryness syndrome should be treated by moistening therapy”comes from Suwen·Zhizhenyaodalun,that is,moist products are used to treat the deficiency of body fluid,and any medicine that can restore the softness of stomach can be called moistening agent.Professor LIU Qingquan,a famous Chinese medicine doctor in China,treated atrophic gastritis with stomach yin deficiency syndrome based on his clinical experience for many years and the theory of“dryness syndrome should be treated by moistening therapy”.In the early stage of CAG,yin was injured by qi stagnation or damp-heat,so sweet and cold regulating qi drugs were used to nourish stomach yin.In the middle stage,because“mother-organ affecting the child-organ”affected the lungs,it is advisable to moisten the yin of lung and stomach with light-hearted drugs.In the later stage,according to the theory that“the five internal organs are connected,and every movement has its own order”,“the injury of the five internal organs must affect the kidney”,salty and cold tonic were used to nourish the yin of stomach and kidney to restore the yin.
