华蟾素胶囊在局部晚期食管鳞癌同步放化疗中的应用价值

摘　　要：目的研究华蟾素胶囊在局部晚期食管鳞癌同步放化疗中的疗效及毒副作用。方法选取局部晚期食管鳞癌患者82例随机分为试验组和对照组,每组41例,对照组采用5氟尿嘧啶和顺铂联合同步放疗,试验组在对照组治疗方案基础上联用华蟾素胶囊。对比观察2组临床缓解率、长期生存情况[无病进展生存期(PFS)、总生存期(OS)]、生活质量及不良反应发生率差异。结果试验组完全缓解率、部分缓解率和疾病稳定率高于对照组,差异有统计学意义(P>0.05)。2组中位PFS中位OS比较,差异无统计学意义(P>0.05)。治疗2个月后,2组KPS评分均较治疗前升高(P>0.05)。试验组治疗后SF-36评分较治疗前显著升高(P<0.05),治疗后试验组SF-36评分高于对照组,差异有统计学意义(P>0.05)。试验组消化道反应、神经毒性反应及肝脏毒性反应发生率较对照组低,差异有统计学意义(P<0.05)。结论局部晚期食管鳞癌患者,在同步放化疗标准治疗方案上联用华蟾素胶囊一定程度上可提高临床疗效,延长PFS及OS,减少不良反应发生率。Objective To investigate the curative effects and safety of cinobufotalin capsule in concurrent chemoradiation in treatment of local advanced esophageal squamous carcinoma.Methods A total of 84 patients with local advanced esophageal squamous carcinoma were randomly divided into two groups,with 41 cases in each group.The patients in control group were treated by 5-fluorouracil,cisplatin combined with concurrent radiotherapy,however,the patients in experimental group,on the basis of control group,were treated by cinobufotalin capsule.The clinical curative effects,patient’s prognosis,life quality and incidence rate of complications were observed and compared between the two groups.Results The complete remission rate partial remission rate and disease stable rate in experimental group were significantly higher than those in control group(P<0.05).There was no significant difference in the median OS of PFS between the two groups(P>0.05).After 2-month treatment,the KPS scores in both groups were significantly increased(P>0.05).And the SF-36 scores in experimental group were significantly higher than those in control group(P<0.05).Moreover the incidence rates of digestive tract reaction,neurotoxicity reaction and liver toxicity in experimental group were significantly lower than those in control group(P<0.05).Conclusion The cinobufotalin capsule combined with concurrent chemoradiation can improve the curative effects on local advanced esophageal squamous carcinoma,which can prolong PFS and OS,and reduce the incidence of adverse reactions.
