开放式和封闭式助萌术在上前牙唇向阻生中的美学意义

摘　　要：目的探究上前牙唇向阻生患者应用封闭式助萌术、开放式助萌术对患者面部美学效果影响。方法62例上颌前牙唇向阻生患者采用随机数字表格法分为观察组和对照组,每组31例。观察组采用封闭式助萌术治疗,对照组31例接受开放式助萌术治疗;比较两种术式美学效果(PES生分),并观察2种术式对边缘骨吸收量影响及牙龈指数(GI)、牙龈出血指数(BI)、菌斑指数(PLI)等影响。结果观察组患者术后PES分值中唇侧龈缘高度与曲线、根部凸度/软组织质地和颜色评分均高于对照组(P<0.05);而2组远中龈乳头、近中龈乳头评分比较,差异无统计学意义(P>0.05);观察组PES分值高于对照组(P<0.05)。2组接受不同术式治疗后,随访显示术后6个月观察组患者边缘骨吸收量,与对照组比较明显减少(P<0.05);随访术后12个月调查显示,2组患者近中、远中骨吸收量比较,差异无统计学意义(P>0.05)。术后12个月与术后6个月组内比较,对照组BI指数、PLI指数术后12个月明显下降,与术后6个月比较,差异有统计学意义(P<0.05),而GI指数术后6个月与12个月比较(P>0.05)。观察组GI、BI、PLI指数术后12个月与6个月比较,差异无统计学意义(P>0.05)。术后6月牙周指数各项指标均低于对照组(P<0.05);而2组术后12个月各数据比较,差异无统计学意义(P<0.05)。观察组患者满意度与观察组比较明显升高(P<0.05)。结论上前牙唇向阻生患者应用封闭式助萌术治疗,相比于开放式助萌术,前者短期牙周指数更低,边缘骨吸收更小,且术后美学效果更佳,有利于提高患者满意度,值得推广。Objective To investigate the effects of closed eruption and open eruption on facial esthetics of patients with labial impaction of maxillary anterior teeth.Methods A total of 62 patients with labial impaction of maxillary anterior teeth who were treated in our hospital from May 2018 to July 2019 were randomly divided into observation group and control group,with 31 cases in each group.The patients in observation group were treated by closed eruption,however,thepatients in control group were treated by open eruption.The aesthetic effects including marginal bone absorption,gingival index(GI),bleeding index(BI),plaque index(PLI)were observed and compared between the two groups.Results The postoperative PES scores of labial gingival margin height and curve,root convexity/soft tissue texture and color score in observation group were significantly higher than those in control group(P<0.05)After 6-month follow up the marginal bone absorption amount in observation group was significantly lower than that in control group(P<0.05),After 12-month follow up there was no significant difference in the bone absorption amount in the proximal,middle and distal regions between the two groups(P>0.05).There were no significant differences in GI,BI,PLI in observation group between 6-month follow up and 12-month follow up(P>0.05).After 6-month follow up,the levels of periodontal indexes in observation group were significantly lower than those in control group(P<0.05),After 12-month follow up,there were no significant differences in the levels of periodontal indexes between the two groups(P>0.05).In addition the satisfaction degree in observation group was significantly higher than that in control group(P<0.05).Conclusion The closed eruption is superior to open eruption in treatment of labial impaction of maxillary anterior teeth,the former has better esthetics effects,with lower short-term periodontal indexes,smaller marginal bone absorption,which is conducive to improve the satisfaction degree of patients,thus,it is worthy of promotio
