摘 要:目的:探讨三花接骨散对股骨骨不连模型大鼠骨形态蛋白7(BMP-7)/转录因子蛋白2(Runx2)信号通路及骨愈合的影响。方法:将60只SD大鼠随机分为正常对照组,模型组,三花接骨散低(1g/kg)、中(4g/kg)、高(8g/kg)剂量组,复方骨肽注射液组(阳性组,25mg/kg),每组10只。除正常对照组外,均建立大鼠股骨骨不连模型。除正常对照组和模型组灌胃给予生理盐水外,三花接骨散组、阳性组均腹腔注射给予相应剂量药物。末次给药12h后,酶联免疫吸附试剂法(ELISA)检测血液血小板衍生因子(PDGF)、血管内皮生长因子(VEGF)水平;取股骨组织,观察股骨大体情况;X线拍片观察骨愈合情况并进行评分;苏木精—伊红染色(HE)检测各组大鼠股骨组织病理变化并进行评分;蛋白免疫印迹法(Western blot)检测骨痂组织中BMP-7、骨钙素(OCN)、Runx2蛋白表达水平。结果:与正常对照组相比,模型组大鼠断端骨性连接较少、间隙较大、骨小梁排列紊乱,X射线评分、骨愈合组织学评分、血清VEGF、PDGF水平及骨组织BMP-7、OCN、Runx2蛋白表达均降低(P<0.05)。与模型组相比,三花接骨散低、中、高剂量组及阳性组大鼠断端间隙减小、骨小梁形成较多,X射线评分、骨愈合组织学评分、VEGF、PDGF水平及BMP-7、OCN、Runx2蛋白表达均升高(P<0.05),且随三花接骨散剂量升高上述指标依次升高。三花接骨散高剂量组与阳性组上述指标差异无统计学意义(P>0.05)。结论:三花接骨散可能通过促进BMP-7/Runx2/OCN信号通路活化,促进骨折处成骨形成,进而改善股骨骨不连大鼠骨愈合状况。Objective:To investigate the effects of Sanhua Jiegu powder on bone morphogenetic protein 7(BMP-7)/transcription factor protein 2(Runx2)signaling pathway and bone healing in rats with femoral nonunion.Methods:60 SD rats were randomly divided into normal control group,model group,low(1g/kg),medium(4g/kg)and high(8g/kg)dose Sanhua Jiegu powder groups,and compound osteopeptide injection group(positive group,25mg/kg),with 10 rats in each group.In addition to the normal control group,the femoral nonunion model was established.Normal control group and model group were given normal saline by gavage.Sanhua Jiegu powder groups and positive group were intraperitoneally injected with corresponding dose of drugs.12 hours after the last administration,the levels of platelet-derived factor(PDGF)and vascular endothelial growth factor(VEGF)were detected by enzyme-linked immunosorbent assay(ELISA);the femoral tissue was taken and the general condition of femur was observed;X-ray film was taken to observe the bone healing and score;hematoxylin eosin staining(HE)was used to detect the pathological changes of femur tissue and score;Western blot was used to detect the protein expression levels of BMP-7,osteocalcin(OCN)and Runx2 in callus tissue.Results:Compared with those in the normal control group,the rats in model group had less bony junction,larger gap and disordered trabecular arrangement,the X-ray score,histological score of bone healing,serum VEGF,PDGF levels and the expression of BMP-7,OCN and Runx2 protein in bone tissue were lower(P<0.05).Compared with those in the model group,the rats in the low,medium and high dose Sanhua Jiegu powder groups and the positive group showed a decrease in the gap between the fracture ends and the formation of bone trabeculae,the X-ray score,histological score of bone healing,VEGF,PDGF levels and the expression of BMP-7,OCN and Runx2 protein were higher(P<0.05),with the increase of Sanhua Jiegu powder,the above indexes increased in turn.There was no significant difference between the high dose
