摘  要:目的:探讨鼻咽部黏膜细胞稳定性检测(FH)联合EB病毒核酸检测(EB-DNA)、癌胚抗原(CEA)在鼻咽癌早期诊断中的价值。方法:采用回顾性研究方法,选取本院耳鼻喉科2018年6月—2020年6月收治的鼻咽癌患者108例作为A组,良性病变组80例作为B组,所有患者经手术后进行病理活检,以病理活检的结果为金标准进行分组。另外选取体检中心80例健康体检者作为C组。三组患者均进行FH检测及血浆EB-DNA、CEA的检测,并分析探讨三项联合检测在鼻咽癌早期诊断中的价值。结果:A组患者的血浆EB-DNA、CEA检测值显著高于B组、C组,B组EB-DNA、CEA明显高于C组,差异有统计学意义(P<0.05);A组的鼻咽部黏膜细胞稳定性FH检测、EB-DNA、CEA检测阳性率明显高于B组,差异有统计学意义(P<0.05);在鼻咽癌患者中三项联合检测的灵敏度、特异度、准确度、阳性预测值、阴性预测值、约登指数可以达到92.60%,91.25%,92.02%,93.46%,90.12%,83.85%,均高于单独检测的值。结论:FH检测联合EB-DNA、CEA在鼻咽癌的早期诊断中具有较高的敏感性和特异性,能提高鼻咽癌的检出率,具有一定的临床应用价值,值得推广。Objective:To explore the value of nasopharyngeal mucosal cell stability test(FH)combined with EB virus nucleic acid test(EB-DNA)and CEA in the early diagnosis of nasopharyngeal carcinoma.Methods:A retrospective study was conducted to select 108 patients with nasopharyngeal carcinoma admitted to the otolaryngology department of our hospital from June 2018 to June 2020 as group A and 80 patients with benign lesions as group B.All patients underwent postoperative pathological biopsy and were grouped according to the gold standard of pathological biopsy results.In addition,80 healthy subjects in the physical examination center were selected as group C.All patients in the three groups were tested by FH and plasma EB-DNA and CEA,and the value of the three combined tests in the early diagnosis of nasopharyngeal carcinoma was analyzed and discussed.Results:The detection values of EB-DNA and CEA in plasma in group A were significantly higher than those in group B and C,and EB-DNA and CEA in group B were significantly higher than those in group C,with statistically significant differences(P<0.05).The positive rates of FH,EB-DNA and CEA in nasopharyngeal mucosa cell stability in group A were significantly higher than those in group B(P<0.05).The sensitivity,specificity,accuracy,positive predictive value,negative predictive value and Yoden index of the three combined tests in nasopharyngeal carcinoma patients can reach 92.60%,91.25%,92.02%,93.46%,90.12%,and 83.85%,all of which are higher than the values of the single test.Conclusion:FH combined with EB-DNA and CEA has high sensitivity and specificity in the early diagnosis of nasopharyngeal carcinoma,and can improve the detection rate of nasopharyngeal carcinoma.It has certain clinical application value and is worthy of promotion.
