术前口服补液盐对乳腺癌根治术患者术中生命体征及术后胃肠功能的影响

摘　　要：目的探究术前口服补液盐(ORS)补液对乳腺癌根治术患者术中生命体征及术后胃肠功能的影响。方法回顾性分析河北北方学院附属第一医院2018年3月~2018年12月收治行乳腺癌根治术患者180例,根据术前口服补液不同将其分为ORS组、清水组和常规组,各60例。观察3组患者生命体征、血糖、不良反应发生情况、住院时间、视觉模拟评分法(VAS)评分及胃肠功能。结果ORS组住院时间、术后胃肠通气时间较清水组和常规组短(P<0.001)。ORS组VAS评分低于清水组和常规组(P<0.001)。T1、T2、T3时段3组SBP、MAP、DBP及HR指标均高于T0,且ORS组各指标均低于清水组和常规组(P<0.01)。T3时段3组血糖、胰岛素及HOMA-IR水平均高于T0(P<0.05),且ORS组显著低于清水组和常规组(P<0.001)。ORS组的不良反应发生率显著低于清水组和常规组(P<0.05)。结论乳腺癌根治术术前ORS补液患者术中生命体征平稳,不良反应发生率低,术后胃肠功能恢复较快,加速患者外科康复。Objective To explore the effect of preoperative administration of oral rehydration salts(ORS)on intraoperative vital signs and postoperative gastrointestinal function in patients undergoing radical mastectomy.Methods 180 patients with radical mastectomy in the First Affiliated Hospital of Hebei North University were selected from March 2018 to December 2018.The patients were divided into ORS group,clear water group and routine group according to the component of oral solution before operation,60 cases in each group.The vital signs,blood sugar,adverse reactions,hospitalization time,score of visual analogue scoring(VAS)and gastrointestinal function were documented.Results The hospitalization time and gastrointestinal exhaust time were shorter in ORS group than those in clear water group and routine group(P<0.001).VAS score was lower in ORS group than that in clear water group and routine group(P<0.001).The systolic blood pressure(SBP),mean arterial pressure(MAP),diastolic blood pressure(DBP)and heart rate(HR)at T1,T2,and T3;were higher than those at T0 in all groups,and were lower in ORS group than those in clear water group and routine group(P<0.01).At T3,the levels of blood glucose,insulin and HOMA-IR in all groups were higher than those at T0(P<0.05),and were lower in ORS group than those in clear water group and the routine group(P<0.001).The incidence of adverse reactions in ORS group was significantly lower than that in clear water group and routine group(P<0.05).Conclusion Stable vital signs,low incidence of adverse reactions,and fast recovery of gastrointestinal function could be achieved by administration of ORS before radical mastectomy.
