右半结肠癌根治术后腹腔淋巴漏的危险因素分析

摘　　要：目的探讨右半结肠癌根治术后腹腔淋巴漏发生的危险因素。方法对461例右半结肠癌根治术后发生腹腔淋巴漏的28例患者,进行相关资料回顾性分析,对其相关危险因素进行单因素及多因素分析。结果右半结肠癌根治术后腹腔淋巴漏经积极非手术治疗有效,未出现需手术治疗患者。12个可能与淋巴漏相关的因素中,低体质量指数(BMI)(P=0.026)、淋巴结转移(P=0.012)、术后非限制饮食(P=0.033)、腹腔镜手术(P=0.002)、动脉导向清扫(P=0.013)均与术后腹腔淋巴漏有关,其中低BMI、动脉导向清扫、术后非限制饮食、腹腔镜手术是术后发生淋巴漏的4个独立高危因素。结论低BMI、动脉导向清扫、术后非限制饮食、腹腔镜手术是右半结肠癌根治术后发生腹腔淋巴漏的独立危险因素。Aim To investigate the risk factors of abdominal lymphorrhagia following radical resection for right colon cancer.Methods Retrospective analysis was carried out for the 28 patients complicated by postoperative abdominal lymphorrhagia among the 461 patients undergoing radical resection for right colon cancerradical.The related risk factors of abdominal lymphorrhagia were analyzed by single factor and Logistic regression analysis.Results Conservative treatment is effective for the patients undergoing abdominal lymphorrhagia following radical resection for right colon cancer,and no patients need the second operation.Among 12 factors possibly related with the abdominal lymphorrhagia,body mass index(BMI)below normal(P=0.026),lymph node metastasis(P=0.012),unlimited diet(P=0.033),laparoscopic operation(P=0.002),arterial guidance(P=0.013)had correlations with abdominal lymphorrhagia following radical resection for right colon cancer;BMI below normal,arterial guidance,unlimited diet,laparoscopic operation were significantly related with the abdominal lymphorrhagia as four dependent high risk factors.Conclusion BMI below normal,arterial guidance,unlimited diet,laparoscopic operation are independent risk factors for the abdominal lymphorrhagia following radical resection for right colon cancer.
