健康体检精细化管理对微小甲状腺高危结节患者的影响

摘　　要：目的分析在健康体检中对微小甲状腺高危结节人群实施精细化管理的影响。方法选取2016年12月至2018年6月在鄂尔多斯中心医院体检中心接受健康体检中发现的微小甲状腺高危结节人群,以患者自愿方式进行精细化管理观察的236例作为研究组,未入组的及于外院进行健康体检的微小甲状腺高危结节人群228例作为对照组。比较两组6个月及1年内复查依从率、结节发生变化后及时手术率及结节进展程度和转移率。结果6个月及1年内,研究组复查依从率明显高于对照组(P<0.05);研究组结节发生变化后及时治疗率明显高于对照组(P<0.05),结节发生变化后进展程度明显小于对照组(P<0.05);研究组转移率明显低于对照组(P<0.05)。结论健康体检中实施精细化管理对微小甲状腺高危结节的复查依从率及对结节发生变化后能及时治疗并减小转移率效果良好,值得临床推广应用。Objective To analyze the influence of fine management on the high risk population of microthyroid nodules in physical examination.Methods Choose between December 2016 and June 2018 in ordos center hospital physical examination center to accept small at high risk of thyroid nodules found in healthy check-up crowd,fine management on the basis of voluntary patients with observation of 236 cases as a team,not into the group and the healthy check-up was conducted in the outer court of tiny people at high risk of thyroid nodules 228 cases as control group.The rate of compliance to review at 6 months and 1 year,the rate of timely operation after changes in nodules,the degree of nodules progression and the rate of metastasis were compared between the two groups.Results At 6 months and 1 year,the compliance rate of the study group was significantly higher than that of the control group(P<0.05).The rate of timely treatment after nodule changes was significantly higher than that in the control group(P<0.05),and the degree of progress after nodule changes was significantly less than that in the control group(P<0.05).The contrast metastasis rate in the study group was significantly lower than that in the control group(P<0.05).Conclusion The implementation of fine management in health examination has a good effect on the reexamination compliance rate of microthyroid nodules with high risk and timely treatment and reduction of metastasis rate after changes in nodules,which is worthy of clinical promotion and application.
