摘  要:目的观察高度屈光参差的近视患者双眼行带散光的有晶体眼人工晶体植入术即TICL后的视功能变化。方法选取2015年6月至2017年12月于本院接受TICL植入术有高度屈光参差的复合性近视散光患者26例,比较患者术前与术后1、6个月的裸眼视力、最佳矫正视力、术后双眼的调节幅度和调节灵敏度的变化。结果所有患者TICL植入超高度近视眼1、6个月的裸眼视力均优于术前最佳矫正视力,差异有统计学意义(P<0.05);中高度近视眼术后1、6个月的裸眼视力与术前最佳矫正视力比较差异无统计学意义。患者双眼调节幅度术前(6.21±2.28)D,术后1个月(7.89±2.58)D,术后6个月(8.26±2.15)D,调节幅度明显增加,分别与术前比较差异有统计学意义(P<0.05);双眼调节灵敏度术前(9.48±1.11)cpm/min,术后1个月(10.16±0.97)cpm/min,术后6个月(10.81±0.57)cpm/min,调节灵敏度明显提高,分别与术前比较差异有统计学意义(P<0.05)。结论高度屈光参差的复合性近视散光患者行TICL植入术能有效降低双眼的屈光度差值,提高患者的裸眼视力、调节幅度和调节灵敏度,改善患者的视觉质量。Objective To observe the changes of visual function after TICL implantation in myopia patients with high anisometropia.Methods 26 patients with compound myopic astigmatism with high anisometropia received TICL implantation in our hospital from June 2015 to December 2017 were selected,the changes of naked eye vision,optimal corrected vision,postoperative adjustment amplitude and adjustment sensitivity of both eyes were compared between preoperative and postoperative 1 month and 6 months.Results The naked eye vision of all patients with TICL implantation was better than the best corrected visual acuity 1 month and 6 months after operation,the difference was statistically significant(P<0.05).There was no statistically significant difference between the naked eye vision 1 month and 6 months after the operation and the best corrected visual acuity before the operation.The patients with binocular adjustment amplitude were(7.89±2.58)D for 1 month and(8.26±2.15)D for 6 months compared with preoperative(6.21±2.28)D.The adjustment amplitude increased and the difference was statistically significant(P<0.05).After the operation of binocular sensitivity adjustment,1 month was(10.16±0.97)cpm/min,and 6 months was(10.81±0.57)cpm/min compared with preoperative(9.48±1.11)cpm/min.The sensitivity of the adjustment was improved and the difference was statistically significant(P<0.05).Conclusion TICL implantation can significantly improve the naked eye vision of patients with compound myopia and astigmatism with high refractive error,improve the adjustment amplitude and sensitivity of patients,and improve the visual quality of patients.
