腹腔镜下子宫全切除术联合盆腔淋巴结清扫术治疗高龄宫颈癌患者的效果观察

摘　　要：目的:探究腹腔镜下子宫全切除术(LTH)联合盆腔淋巴结清扫术(PLND)在高龄宫颈癌治疗中的效果。方法:选取高龄宫颈癌患者86例作为研究对象,按照随机数字表法分为两组。开腹组(42例)接受传统开腹根治性子宫全切除术治疗,微创组(44例)接受LTH联合PLND治疗,观察并比较两组手术时间、术中出血量、淋巴结清扫数、住院时间、拔管时间、首次排便时间、首次排气时间及并发症状况。结果:微创组手术时间长于开腹组,手术出血量少于开腹组,住院时间、拔管时间、首次排便时间、首次排气时间短于开腹组,两组比较差异有统计学意义(P<0.05);微创组并发症发生率6.82%低于开腹组28.57%,两组比较差异有统计学意义(P<0.05)。结论:LTH联合PLND治疗高龄宫颈癌,可优化手术效果,加快康复进程,提高安全性。Objective:To investigate the effects of laparoscopic total hysterectomy(LTH)combined with pelvic lymph node dissection(PLND)on the treatment of elderly patients with cervical cancer.Methods:86 elderly patients with cervical cancer were randomized into 2 groups,42 cases in the open surgery group received conventional open radical hysterectomy,while another 44 cases in minimally invasive group underwent LTH combined with PLND treatment.Then duration of operation,amount of blood loss,number of lymph nodes dissected,length of hospital stays,time of extubation,time of first defecation,time of first exhaust and the status of complications of patients were compared between both groups.Results:Compared with the open surgery group,the duration of operation and amount of blood loss were less,and the length of hospital stays,time of extubation,time of first defecation,time of first exhaust were shorter,which were of statistical differences(P<0.05);and the incidence of complications in the minimally invasive group was 6.82%,which was lower than 28.57%in the open surgery group,and the difference was significant(P<0.05).Conclusion:LTH combined with PLND treatment of senile cervical cancer can optimize the surgical effects,speed up the rehabilitation process,and improve safety of surgeries.
