摘 要:目的目的分析并总结散发型克雅氏病脑18F-fluorodeoxyglucose-positron emission tomography(18F-FDG PET)的影像学特点,以探讨并寻找可靠的影像学指标如典型的区域代谢变化用于散发型克雅氏病的辅助诊断。方法收集郑州大学第一附属医院自2017年1月至2019年12月间临床诊断为散发型克雅氏病的患者,总结散发型克雅氏病患者PET和MRI的影像学特点,分析PET与MRI检查结果的差异,探讨PET检查对于散发型克雅氏病早期诊断的可能性。结果共收集8例临床诊断的很可能以及可能的散发型克雅氏病患者,其中6例患者PET结果异常,主要表现为基底节区、额叶、顶叶、颞叶等部位代谢降低,2例患者MRI结果异常,主要表现为基底节区、额顶叶T2高信号。4例MRI正常的患者PET结果异常。2例PET和MRI均异常的患者中,1例PET的异常部位多于MRI异常部位,1例PET异常部位与MRI一致。结论在散发型克雅氏病的早期阶段,PET异常可早于MRI出现,且异常部位多于MRI;相较于MRI,PET可能具备更高的灵敏度,可为散发型克雅氏病早期诊断的有效手段。Objective 18 F-fluorodeoxyglucose-positron emission tomography(18 F-FDG PET)scanning may have advantages for early diagnosis of sporadic Creutzfeldt-Jakob disease(sCJD),as it may reveal cellular glucose transport and metabolism changes in the cortex,cerebellum,basal ganglia and other parts.The aim of the present study was to summarize characteristics of 18 F-FDG PET and explore reliable imaging markers,such as regional metabolic changes,to apply to make a diagnosis of sCJD.Methods We retrospectively collected cases diagnosed with sCJD between January 2017 and December 2019 at First Affiliated Hospital of Zhengzhou University,and analyzed the patients’18 F-FDG PET and MRI results.Results We collected 8 patients diagnosed with probable or sCJD who went through PET.Among them,6 patients‘PET results presented abnormalities,mainly manifested as bilatera basal ganglia,frontal,parietal and temporal lobes hypometabolism.2 patients had abnormal MRI results,mainly manifested as high signal intensity on T2-weighted images and DWI images.4 patients with no abnormal MRI findings had hypometabolism on PET.1 patients’PET imaging showed more abnormal regions than MRI.Conclusion In the early stage of sCJD,PET abnormalities can appear earlier than MRI,and show more abnormal regions than MRI.Compared with MRI,PET may have higher sensitivity and can detect sCJD at an earlier stage.
