生血宝合剂联合复方硫酸亚铁叶酸片治疗妊娠合并地中海贫血的效果及对患者妊娠结局的影响

摘　　要：目的分析生血宝合剂联合复方硫酸亚铁叶酸片治疗妊娠合并地中海贫血的临床效果及对患者妊娠结局的影响。方法64例妊娠合并地中海贫血患者,按抽签法分为生血宝合剂组与联合组,各32例。生血宝合剂组服用生血宝合剂治疗,联合组在生血宝合剂组治疗基础上口服复方硫酸亚铁叶酸片治疗。比较两组血常规参数[平均红细胞体积(MCV)、红细胞计数(RBC)、血红蛋白(Hb)、血清铁蛋白(FER)]、中医症候积分及妊娠结局。结果治疗后,两组MCV、FER、Hb、RBC水平均高于本组治疗前,且联合组MCV、FER、Hb、RBC水平分别为(78.50±3.49)fl、(28.08±8.49)ng/ml、(108.54±4.20)g/L、(4.32±2.48)×10^(12)/L,均高于生血宝合剂组的(72.53±4.91)fl、(13.53±4.91)ng/ml、(91.96±4.86)g/L、(3.01±2.51)×10^(12)/L,差异均具有统计学意义(P<0.05)。治疗后,两组面色萎黄、潮热盗汗、心悸气短积分均低于本组治疗前,且联合组面色萎黄、潮热盗汗、心悸气短积分分别为(1.64±0.49)、(1.48±0.45)、(1.31±0.43)分,均低于生血宝合剂组的(2.85±0.50)、(2.36±0.58)、(2.16±0.52)分,差异均具有统计学意义(P<0.05)。联合组妊娠期高血压、产后出血、胎儿窘迫、低体重儿、新生儿窒息发生率均低于生血宝合剂组,差异均具有统计学意义(P<0.05)。结论生血宝合剂联合复方硫酸亚铁叶酸片可改善妊娠合并地中海贫血患者妊娠结局,减轻不适反应,改善血常规,有应用意义。Objective To analyze the clinical effect of Shengxuebao mixture combined with compound ferrous sulfate and folic acid tablets in the treatment of pregnancy complicated with thalassemia and its influence on the pregnancy outcome.Methods A total of 64 cases of pregnancy complicated with thalassemia were divided into Shengxuebao mixture group and combined group by lottery method,with 32 cases in each group.Shengxuebao mixture group was treated with Shengxuebao mixture,and combined group was treated with oral compound ferrous sulfate and folic acid tablets based on Shengxuebao mixture group.The blood routine parameters[mean corpuscular volume(MCV),red blood cell count(RBC),hemoglobin(Hb),serum ferritin(FER)],traditional Chinese medicine syndrome score and pregnancy outcome were compared between the two groups.Results After treatment,the MCV,FER,Hb and RBC of the two groups were higher than those before treatment,and the MCV,FER,Hb and RBC of the combined group were(78.50±3.49)fl,(28.08±8.49)ng/ml,(108.54±4.20)g/L and(4.32±2.48)×10^(12)/L,which were higher than(72.53±4.91)fl,(13.53±4.91)ng/ml,(91.96±4.86)g/L and(3.01±2.51)×10^(12)/L of Shengxuebao mixture.All the difference was statistically significant(P<0.05).After treatment,the yellow complexion,hot flashes,night sweats,palpitations,shortness of breath score of the two groups were lower than those before treatment,and yellow complexion,hot flashes,night sweats,palpitations,shortness of breath score of the combined group were(1.64±0.49),(1.48±0.45)and(1.31±0.43)points,which were lower than(2.85±0.50),(2.36±0.58)and(2.16±0.52)points of Shengxuebao mixture group.All the difference was statistically significant(P<0.05).The incidences of pregnancy hypertension,postpartum hemorrhage,fetal distress,low weight infants,neonatal asphyxia of the combined group were lower than those of Shengxuebao mixture group,and the difference was statistically significant(P<0.05).Conclusion Combination of Shengxuebao mixture and compound ferrous sulfate and folic acid table
