老年人胰腺癌组织中HER2表达及临床意义

摘　　要：目的探讨老年人胰腺导管腺癌中人类上皮生长因子受体2(HER2)的表达及与临床病理特征的关系。方法收集老年人胰腺导管腺癌67例,相对应的淋巴结转移灶20例和老年人正常胰腺组织16例,应用免疫组化Envision二步法检测HER2蛋白的表达情况,根据Hercep Test评分标准对HER2表达进行评定,并分析其与临床病理特征的关系。结果老年人胰腺导管腺癌组织中HER2过表达率为38.8%,正常胰腺组织中HER2无过表达。老年人胰腺导管腺癌与正常胰腺组织HER2过表达率差异有统计学意义(P=0.003)。老年男性的胰腺导管腺癌HER2过表达率明显高于老年女性,差异有统计学意义(P=0.043),但老年人胰腺导管腺癌HER2过表达率与肿瘤部位、肿瘤分级、T分期、N分期、神经侵犯、切缘阳性无明显相关性(P>0.05)。老年人胰腺导管腺癌淋巴结转移灶中HER2表达与原发灶无明显相关性(P>0.05)。结论HER2在老年人胰腺导管腺癌的发生发展过程中可能起重要作用,老年患者的胰腺导管腺癌HER2过表达率可能与性别有关,其机制尚需进一步研究。Objective To investigate the expression of human epidermal growth factor receptor 2(HER2)in pancreatic carcinoma tissue in elderly patients with pancreatic ductal adenocarcinoma(PDAC),and to explore its correlation with clinicopathological features of PDAC.Methods A total of 67 elderly patients with PDAC,67 cases of PDAC with lymphaden metastasis and 16 cases of normal pancreatic tissue were enrolled in the study.The expression levels of HER2 protein were detected by immunohistochemical Envision two-step method,and the immuno-reactivity was evaluated according to the Hercep Test Scoring Guideline,and the correlation between HER2 expression and clinicopathological features was analyzed.Results The overexpression rate of HER2 in elderly patients with PDAC was 38.8%,but there was no overexpression in normal pancreatic tissue.The overexpression rate of HER2 in patients with PDAC was significantly higher than that in normal pancreatic tissue(P<0.01).The overexpression rate of HER2 in elderly male patients was significantly higher than that in elderly female patients(P<0.05),however,which was no correlated with tumor location,tumor grade,T stage,N stage,nerve invasion and positive margin(P>0.05).There was no significant correlation between HER2 expression in lymph node metastasis and primary lesion in elderly patients with PDAC(P>0.05).Conclusion The expression of HER2 may play an important role in the pathogenesis and development of PDAC in the elderly.The overexpression rate of HER2 in elderly patients with PDAC may be related to patient’s gender,however,its action mechanism needs to be researched further.
