PET/CT与MRI诊断原发性肝细胞癌介入治疗后边缘复发灶的对比研究

摘　　要：目的对比PET/CT与MRI诊断原发性肝癌(HCC)介入治疗后边缘复发灶的应用价值。方法收集2015年10月~2019年12月在西部战区总医院治疗的肝癌患者62例,经导管动脉化疗栓塞术(TACE)治疗6个月后进行影像和AFP随访。每例均行PET/CT和MRI检查,边缘病灶定性以穿刺病理结果为金标准。比较两种影像检查的灵敏度、特异度、准确度。结果PET/CT图像的准确度为91.9%,灵敏度为97.8%,均明显高于MRI(准确度79.0%、灵敏度86.7%)(P<0.05);其中,PET/CT图像确诊的患者AFP水平为(12.6±2.7)ng/mL,显著低于MRI图像(16.7±4.3)ng/mL(P<0.05)。结论在肝癌TACE治疗后随访中,PET/CT较MRI有更高的灵敏度和准确度,能更早发现边缘复发病灶,对患者预后十分有利,可在临床推广应用。Objective To compare the value of PET/CT and MR in the diagnosis of marginal recurrence of hepatocellular carcinoma after(HCC)interventional therapy.Methods 62 patients with liver cancer treated in Western Theater General Hospital from October 2015 to December 2019 were followed up by imaging and AFP after 6 months of TACE treatment.PET/CT and MR were performed in each case,and the qualitative diagnosis of marginal lesions was based on the pathological results of puncture as the gold standard.The sensitivity,specificity and accuracy of the two kinds of imaging examination were compared.Results The accuracy and sensitivity of PET/CT group were 91.9%and 97.8%respectively,which were significantly higher than those of MR group(79.0%and 86.7%)(P<0.05).Among them,the AFP level of patients diagnosed in PET/CT group was significantly lower than that of MR group 16.7±4.3 ng/ml(P<0.05).Conclusion In the follow-up of liver cancer after TACE treatment,PET/CT has higher sensitivity and accuracy than MR,and can find the focus earlier,which is very beneficial to the prognosis of patients,which is worthy of clinical follow-up.
