改良隧道技术在治疗多牙位Miller Ⅲ度牙龈退缩中的应用

摘　　要：目的评价改良隧道技术(Tunnel)联合上皮下结缔组织(connective tissue graft, CTG)移植治疗多牙位MillerⅢ度牙龈退缩的疗效。方法选取10例多牙位MillerⅢ度牙龈退缩患者共40颗患牙纳入研究。在术前和手术后6个月分别对患牙的牙龈退缩深度、角化龈宽度、探诊深度、临床附着水平和邻接点至牙龈乳头顶点的距离进行评估记录,并计算平均根面覆盖率和完全根面覆盖率。结果手术后6个月,牙龈退缩深度从(3.08±0.94) mm降低至(0.70±0.69) mm,角化龈宽度增宽了(0.25±0.49) mm,临床附着水平及邻接点至牙龈乳头顶点的距离均有显著改善;平均根面覆盖率达79.78%,完全根面覆盖率为42.50%。进一步分析显示,平均根面覆盖率和完全根面覆盖率与患牙的上、下颌位及牙位数无关。结论使用改良Tunnel联合CTG治疗多牙位MillerⅢ度牙龈退缩可获得良好的临床疗效,是多牙位MillerⅢ度牙龈退缩的有效治疗方法之一。Objective To evaluate the effect of modified Tunnel technique with connective tissue graft(CTG) on the treatment of multiple Miller Ⅲ gingival recessions. Methods A total of 40 teeth from 10 patients were included in this study. The gingival recessive depth(REC), keratinized gingival width(KGW), probing depth(PD), clinical attachment level(CAL) and the distance between the contact point and the tip of the papilla(DCP) were evaluated and recorded before and 6 months after root coverage surgery. The mean root coverage and complete root coverage were also calculated. Results Six months after surgery, REC decreased from(3.08±0.94) mm to(0.70±0.69) mm, the KGW increased(0.25±0.49) mm, and CAL and DCP were also significantly improved. The mean root coverage was 79.78% and the complete root coverage was 42.50%. Further analysis showed that mean root coverage and complete root coverage were not related to the teeth position or the number of adjacent teeth. Conclusion Modified Tunnel technique combined with CTG can achieve good clinical effect, which may be an effective method for treatment of multiple Miller Ⅲ gingival recessions.
