不同激光经尿道切除非肌层浸润性膀胱癌的疗效比较

摘　　要：目的比较分析经尿道铥激光和钬激光膀胱肿瘤切除术治疗非肌层浸润性膀胱癌的安全性和临床疗效。方法回顾性分析2014年1月~2017年7月哈尔滨医科大学附属第一医院收治的160例非肌层浸润性膀胱癌患者临床资料,根据手术方式的不同将入选患者分成铥激光组(经尿道铥激光膀胱肿瘤切除术,n=80)及钬激光组(经尿道钬激光膀胱肿瘤切除术,n=80)。观察入选患者的手术时间、术中出血情况、术后膀胱冲洗时间、留置导尿管时间、术后住院周期、术中及术后并发症发生率、复发率及生活质量水平变化情况。结果两组入选患者的性别及年龄构成、手术时间、并发症发生率和术后半年复发率等数据的差异无明显统计学意义(P>0.05)。术中出血情况、术后持续膀胱冲洗时间、留置尿管时间、术后住院周期及一年复发率,铥激光组均明显优于钬激光组(P<0.05);两组患者接受治疗后,其生活质量与治疗前比较各项评分均得到明显提升,具有统计学意义(P<0.05),且铥激光组取得的生活质量评分均明显优于钬激光组,差异具有统计学意义(P<0.05)。结论与经尿道钬激光手术相比,经尿道铥激光切除术治疗膀胱肿瘤在经济及疗效上更有优势,值得推广。Objective To compare the safety and clinical efficacy of transurethral thulium laser resection of bladder tumor and transurethral holmium laser resection of bladder tumor in the treatment of non-muscle-invasive bladder cancer. Methods The clinical data of 160 patients with non-muscle-invasive bladder cancer admitted to the First Affiliated Hospital of Harbin Medical University were analyzed retrospectively. The patients were selected from January 2014 to July 2017. According to the differences of clinical interventions, the patients were divided into thulium laser group(transurethral thulium laser resection of bladder cancer, n=80) and holmium laser group(transurethral holmium laser resection of bladder cancer, n=80). The operation time, intraoperative bleeding volume, bladder irrigation time, indwelling catheter time, hospitalization time, incidence of complications, recurrence rate and quality of life were observed. Results There was no significant difference between the two groups in general data such as sex and age, and there was no significant difference between the two groups in terms of operation time and complication rate(P>0.05). The bleeding volume, bladder irrigation time, indwelling catheter time, hospitalization time and one-year recurrence rate of the thulium laser group was significantly better than the holmium laser group(P<0.05). The quality of life evaluation indicators of thulium laser group were significantly improved, and the difference of data was significant(P<0.05). Compared with holmium laser group, the quality of life score of thulium laser group was significantly better than that of holmium laser group(P<0.05). Conclusion Compared with transurethral holmium laser surgery, transurethral thulium laser bladder tumor resection has more advantages in economic and therapeutic effect. It is worth promoting.
