摘 要:目的利用MRI技术对颊侧多曲簧活动矫治器治疗的AngleⅢ患儿的颞下颌关节盘进行扫描,比较矫治前后患儿颞下颌关节盘的改建情况。方法选择10例于哈尔滨医科大学附属第二医院口腔正畸科就诊的,年龄在8~11岁之间的患儿作为研究对象。患儿均为反(牙合)且无颞下颌关节症状。应用Phillips Achieva3.0T超导体磁共振成像系统及头部NV线圈进行T1WI序列的斜矢状位扫描,记录扫描结果,测量比较矫治前后颞下颌关节盘厚度改建的变化情况。结果在斜矢状位,(1)牙尖交错位时,关节盘前、中、后带的厚度从矫治前(2.40±0.62)mm、(1.00±0.16)mm、(2.68±0.72)mm,变化为矫治后(2.78±0.57)mm、(0.80±0.15)mm、(2.35±0.65)mm,P<0.01,差异有统计学意义。(2)最大开口位的关节盘前、中、后带的厚度从矫治前(2.55±0.63)mm、(1.35±0.49)mm、(3.43±0.91)mm,变化为矫治后(3.04±0.92)mm、(1.08±0.45)mm、(3.12±0.96)mm,P<0.05,差异有统计学意义。结论颊侧多曲簧矫治器可以使替牙期AngleⅢ类错(牙合)的关节盘形态发生改变,前带厚度增大,中带、后带厚度减小,具有较高的临床矫治效能。Objective To compare the temporomandibular joint(TMJ) disc of children with Angle Ⅲ patients treated with buccal multiloop appliance before and after the treatment by MRI. Methods Ten patients aged from 8 to 11 years old with mandibular protrusion, from the Department of Orthodontics of the Second Affiliated Hospital of Harbin Medical University were chosen. All the patients were diagnosed as Angle Ⅲ without temporal-mandibular joint symptoms. All the cases were scanned with a Phillips Achieva3.0 T MRI systems and a head NV coil for the axial, oblique sagittal and oblique coronal scans. The changes of temporal-mandibular joint thickness were measured and compared before and after the correction. Results(1)At the intercuspal position: before the correction, the articular disc the anterior band, medial band and posterior band thickness were(2.40±0.62)mm,(1.00±0.16)mm, and(2.68±0.72)mm, respectively. After the correction, they were(2.78±0.57)mm,(0.80±0.15)mm, and(2.35±0.65)mm, respectively. There were significant differences between the two groups.(2)At the maximum open position, before the correction, the thicknesses of the articular disc anterior band, medial band and posterior band were(2.55±0.63)mm,(1.35±0.49)mm, and(3.43±0.91)mm, respectively;After the correction, the thicknesses were(3.04±0.92)mm,(1.08±0.45)mm, and(3.12±0.96)mm, respectively. There were significant differences between the two groups. Conclusion Patients with Angle Ⅲ treated with buccal multiloop appliance whose the articular disc morphological changes: the thickness of anterior band increases, and the thickness of medial band and the posteriorband decrease. It has a high clinical therapeutic efficacy.
