摘 要:目的研究妊娠早期开始规律瑜伽运动是否可有效降低前次妊娠血糖异常妇女再次妊娠时妊娠期糖尿病(GDM)发生率。方法选取经产妇500例,前次妊娠诊断GDM或妊娠期糖耐量受损,随机分为瑜伽组和对照组,各250例。从早孕期开始,对照组孕妇保持平日的体力活动水平,而瑜伽组孕妇在专业瑜伽教练的看护和指导下每周进行3次瑜伽训练,每次1 h。经过16~18周的运动干预后,瑜伽组和对照组孕妇分别完成75 g口服葡萄糖耐量试验。比较2组孕妇26周前体质量增长情况、糖化血红蛋白、应用胰岛素比例等。结果对照组孕前体型正常孕妇、超体质孕妇及肥胖孕妇的26周体质量增长多于瑜伽组。差异有统计学差异(P<0.05)。对照组GDM发生率高于瑜伽组,差异有统计学意义(P<0.05)。瑜伽组OGTT各点血糖值均低于对照组,差异有统计学意义(P<0.05)。结论对于前次妊娠血糖异常的妇女,再次妊娠早期规律瑜伽运动,可有效控制孕期体质量增长,减少妊娠期糖尿病的发生。Objective To study whether regular yoga exercise in early pregnancy can effectively reduce the incidence of gestational diabetes mellitus(GDM)in women with abnormal blood glucose in previous pregnancy.Methods A total of 500 multipara women were randomly divided into yoga group and control group,250 cases in each group.From early pregnancy,the control group maintained the level of daily physical activity,while the yoga group received yoga training three times a week,one hour each time,under the care and guidance of a professional yoga instructor.After 16-18 weeks of exercise intervention,pregnant women in yoga group and control group completed 75g oral glucose tolerance test respectively.The growth of precursor weight,glycosylated hemoglobin and the proportion of insulin application were compared between the two groups at 26 weeks.Results In the control group,the weight gain at 26 weeks and the weight gain during pregnancy were higher in the control group than in the yoga group,and the difference was statistically significant(P<0.05).The incidence of GDM in the control group was higher than that in the yoga group,and the difference was statistically significant(P<0.05).The blood glucose of OGTT in yoga group was lower than that in control group,and the difference was statistically significant(P<0.05).Conclusion For women with abnormal blood glucose in the previous pregnancy,regular yoga exercise in early pregnancy can effectively control the weight gain during pregnancy and reduce the incidence of gestational diabetes mellitus.
