摘　　要：目的检测胃癌组织中Piwi蛋白相互作用RNA-823(piRNA-823)的表达情况,分析其临床意义。方法选取76例胃癌患者的胃癌组织及癌旁正常组织(距肿瘤组织﹥5 cm)标本,检测piRNA-823表达水平,分析其与患者临床特征的关系以及对远处转移的影响。结果76例胃癌患者胃癌组织中piRNA-823表达水平为(2.34±0.68),低于癌旁正常组织的(3.51±0.71),差异有统计学意义(P﹤0.05)。以胃癌组织中piRNA-823检测值﹤癌旁正常组织的患者(50例)为低表达组,另外26例为高表达组。piRNA-823低表达组患者的总生存期短于高表达组,差异有统计学意义(P﹤0.05)。piRNA-823高表达与低表达胃癌患者浸润深度、淋巴结转移情况、TNM分期比较,差异均有统计学意义(P﹤0.05)。多因素分析显示,TNM分期是胃癌侵袭转移的影响因素(P﹤0.01),而piRNA-823表达情况非胃癌侵袭转移的影响因素(OR=1.01,95%CI:0.89~3.98)。结论piRNA-823在胃癌组织中低表达,与肿瘤的抑制作用有关,可当作胃癌的诊断指标之一以及靶向治疗的依据。Objective To investigate the expression of Piwi-interacting RNA-823(piRNA-823)in gastric cancer and its clinical significance.Method A total of 76 patients with gastric cancer were selected.Their tumor tissues and normal adjacent tissues(>5 cm from the tumor tissue)were collected.The level of piRNA-823 was measured,and the relationship between piRNA-823 and clinical characteristics,including distant metastasis,was analyed.Result The expression level of piRNA-823 in gastric cancer tissues of 76 patients with gastric cancer was(2.34±0.68),which was lower than that of normal adjacent tissue(3.51±0.71),and the difference was statistically significant(P<0.05).Patients with piRNA-823 value in gastric cancer tissues lower than normal adjacent tissue(50 cases)were set as a low-expression group,the other 26 cases were established as a high-expression group.The overall survival of patients in the low-expression group was shorter than that in the high-expression group,and the difference was statistically significant(P<0.05).In addition,there were statistically significant differences in the depth of invasion,lymph node metastasis,and the TNM stage between patients with piRNA-823 high-and low-expression(P<0.05).Multivariate analysis indicated that the TNM stage was an independent risk factor of gastric cancer invasion and metastasis(P<0.01),while piRNA-823 expression was not an independent risk factor of gastric cancer invasion and metastasis(OR=1.01,95%CI:0.89-3.98).Conclusion The down-regulation of piRNA-823 in gastric cancer tissues is related to tumor suppression,such an indicator could be used as one of the diagnostic markers of gastric cancer and prepared for further targeted therapy.
