摘  要:目的探讨腺苷脱氨酶(ADA)在肝脏疾病中的临床应用。方法184例肝脏疾病患者,按照疾病类型分为急性肝炎组(62例)、酒精性肝炎组(26例)、慢性肝炎组(57例)、重型肝炎组(22例),肝硬化组(17例),另选取同期70例体检健康者作为对照组,检测六组腺苷脱氨酶、胆碱酯酶、前清蛋白水平。比较六组血清指标(腺苷脱氨酶、胆碱酯酶、前清蛋白)水平以及血清指标异常情况。结果急性肝炎组、酒精性肝炎组、慢性肝炎组、重型肝炎组、肝硬化组、对照组的腺苷脱氨酶分别为(32.84±10.62)、(37.54±11.68)、(22.13±12.17)、(37.02±8.46)、(42.56±7.03)、(10.43±3.74)U/L,胆碱酯酶分别为(3824.62±2337.26)、(5103.27±1982.35)、(7322.16±2075.63)、(2753.35±1119.54)、(1899.35±143.42)、(8366.48±1907.28)U/L,前清蛋白分别为(196.62±44.45)、(220.98±47.12)、(195.72±40.67)、(143.08±53.64)、(120.35±45.89)、(325.10±56.31)mg/L。急性肝炎组、酒精性肝炎组、慢性肝炎组、重型肝炎组、肝硬化组的腺苷脱氨酶较对照组均有不同程度的升高,胆碱酯酶、前清蛋白较对照组均有不同程度的降低,差异均具有统计学意义(P<0.05)。各组腺苷脱氨酶异常率由高到低:肝硬化组>重型肝炎组>酒精性肝炎组>急性肝炎组>慢性肝炎组;各组胆碱酯酶异常率由高到低:肝硬化组>重型肝炎组>急性肝炎组>酒精性肝炎组>慢性肝炎组;各组前清蛋白异常率由高到低:慢性肝炎组>肝硬化组>重型肝炎组>酒精性肝炎组>急性肝炎组。六组腺苷脱氨酶、胆碱酯酶、前清蛋白异常率比较差异具有统计学意义(P<0.05)。结论对肝脏疾病患者进行腺苷脱氨酶的检测,可较早的反映肝脏的合成功能、肝实质细胞的功能状态和损伤程度,是评价肝脏疾病患者病情及判断预后的重要指标,具有临床参考价值。Objective To discuss the clinical application of adenosine deaminase(ADA)in liver diseases.Methods A total of 184 patients with liver disease were divided into acute hepatitis group(62 cases),alcoholic hepatitis group(26 cases),chronic hepatitis group(57 cases),severe hepatitis group(22 cases),and liver cirrhosis group(17 cases)according to the types of disease.Another 70 healthy subjects were taken as the control group.The levels of adenosine deaminase,cholinesterase and prealbumin in each group were detected.The serum indicators(adenosine deaminase,cholinesterase,prealbumin)and abnormal serum indicators of the six groups were compared.Results The adenosine deaminase of acute hepatitis group,alcoholic hepatitis group,chronic hepatitis group,severe hepatitis group,liver cirrhosis group and control group were(32.84±10.62),(37.54±11.68),(22.13±12.17),(37.02±8.46),(42.56±7.03)and(10.43±3.74)U/L,the cholinesterase were(3824.62±2337.26),(5103.27±1982.35),(7322.16±2075.63),(2753.35±1119.54),(1899.35±143.42)and(8366.48±1907.28)U/L,and the prealbumin were(196.62±44.45),(220.98±47.12),(195.72±40.67),(143.08±53.64),(120.35±45.89)and(325.10±56.31)mg/L.The adenosine deaminase of acute hepatitis group,alcoholic hepatitis group,chronic hepatitis group,severe hepatitis group,liver cirrhosis group were higher than those of the control group,and cholinesterase and prealbumin were lower than those of the control group.All the difference was statistically significant(P<0.05).The abnormal ratio of adenosine deaminase in each group was from high to low:liver cirrhosis group>severe hepatitis group>alcoholic hepatitis group>acute hepatitis group>chronic hepatitis group;the proportion of abnormal cholinesterase in each group was from high to low:liver cirrhosis group>severe hepatitis group>acute hepatitis group>alcoholic hepatitis group>chronic hepatitis group;the abnormal ratio of prealbumin in each group was from high to low:chronic hepatitis group>liver cirrhosis group>severe hepatitis group>alcoholic hepatitis grou
