牙隐裂伴牙髓炎患者根管治疗后全瓷冠修复和金属烤瓷冠修复的效果比较

摘　　要：目的比较牙隐裂伴牙髓炎患者根管治疗后全瓷冠修复和金属烤瓷冠修复的效果。方法60例牙隐裂伴牙髓炎患者,以随机数字表法分为观察组和对照组,各30例。所有患者均行根管治疗后,对照组患者通过金属烤瓷冠修复,观察组患者通过全瓷冠修复。比较两组患者的疼痛评分、菌斑指数、牙周探诊深度、生活质量评分以及治疗效果。结果两组患者的疼痛评分比较差异无统计学意义(P>0.05);观察组患者菌斑指数(0.8±0.5)低于对照组的(1.7±0.7),牙周探诊深度(2.0±1.1)mm小于对照组的(2.7±1.0)mm,差异具有统计学意义(P<0.05)。观察组患者的治疗总有效率为96.7%(29/30),高于对照组的73.3%(22/30),差异具有统计学意义(P<0.05)。观察组患者的总体健康评分为(74.16±3.85)分,生理功能评分为(71.46±4.68)分,心理健康评分为(67.59±4.67)分,身体疼痛评分(65.71±4.48)分,情感职能评分为(75.42±4.49)分,社会功能评分为(65.18±4.42)分,均高于对照组的(61.43±4.28)、(55.87±4.47)、(55.19±3.56)、(54.35±5.47)、(66.24±2.57)、(55.72±4.62)分,差异具有统计学意义(P<0.05)。结论牙隐裂伴牙髓炎患者通过全瓷冠修复根管治疗,其治疗效果要优于金属烤瓷冠修复的治疗效果,患者的生活质量可以得到显著改善,值得推广。Objective To compare the effect of all-ceramic crown restoration and metal-ceramic crown restoration in patients with cracked tooth and pulpitis after root canal treatment.Methods A total of 60 patients with cracked tooth and pulpitis were divided into observation group and control group according to random numerical table,with 30 cases in each group.After root canal treatment,the control group was treated with metal-ceramic crown restoration,and the observation group was treated with all-ceramic crown restoration.The pain score,plaque index,periodontal probing depth,quality of life and therapeutic effect were compared between the two groups.Results There was no statistically significant in pain score between the two groups(P>0.05).The plaque index(0.8±0.5)of the observation group was lower than(1.7±0.7)of the control group,and periodontal probing depth(2.0±1.1)mm was less than(2.7±1.0)mm of the control group.All the difference was statistically significant(P<0.05).The total effective rate 96.7%(29/30)of the observation group was higher than 73.3%(22/30)of the control group,and the difference was statistically significant(P<0.05).The overall health score(74.16±3.85)points,physiological function score(71.46±4.68)points,mental health score(67.59±4.67)points,body pain score(65.71±4.48)points,emotion function score(75.42±4.49)points and social function score(65.18±4.42)points of the observation group were higher than(61.43±4.28),(55.87±4.47),(55.19±3.56),(54.35±5.47),(66.24±2.57)and(55.72±4.62)points of the control group,and the difference was statistically significant(P<0.05).Conclusion All-ceramic crown restoration for the patients with cracked tooth and pulpitis has better therapeutic effect than that of metal-ceramic crown restoration,and it can significantly improve the quality of life of patients,which is worth popularizing.
