合并焦虑对子宫内膜癌根治术后患者的康复情况、免疫功能及血清肿瘤标志物的影响

摘　　要：目的探讨合并焦虑对子宫内膜癌根治术后患者的恢复、免疫功能及血清肿瘤标志物的影响。方法选取我院146例接受过子宫内膜癌根治术的子宫内膜癌患者,根据术后3个月焦虑情况分为焦虑组和非焦虑组,各73例,观察3个月后两组康复情况、自我护理能力(ESCA)、血清肿瘤标志物[糖类抗原125(CA125)、人附睾分泌蛋白4(HE4)]及免疫功能指标[CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+),CA125、HE4]水平、疼痛程度(VAS)和生活质量(SF-36)。结果焦虑组患者康复情况低于非焦虑组(P<0.05),ESCA评分低于非焦虑组(P<0.05),两组CA125、HE4、CD3^(+)、CD4^(+)、CD8^(+)水平比较差异无统计学意义(P>0.05),焦虑组CD4^(+)/CD8^(+)水平低于非焦虑组(P<0.05),VAS评分高于非焦虑组(P<0.05),SF-36低于非焦虑组(P<0.05)。结论合并焦虑会减慢患者子宫内膜根治术术后恢复速度,使患者机体免疫功能失调、降低患者生活质量,因此临床上要多注意患者术后心理状况,针对患者情况采取适当心理干预措施。Objective To study the effect of anxiety on postoperative recovery,immune function and serum tumor markers of endometrial carcinoma.Methods The total of 146 patients with endometrial cancer who had undergone radical hysterectomy in our hospital were selected,The patients were divided into anxiety group(n=73)and non anxiety group(n=73),After 3 months,the recovery,self-care ability(ESCA),serum tumor markers(CA125,HE4),immune function indexes(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+),CA125、HE4),pain degree(VAS)and quality of life(SF-36)between two group were observed.Results There was no significant difference in the levels of CA125,HE4,CD3^(+),CD4^(+),CD8^(+)between the two groups(P>0.05).The level of CD4^(+)/CD8^(+)in anxiety group was significantly lower than that in non anxiety group(P<0.05),VAS score was significantly higher than that in non anxiety group(P<0.05),and SF-36 was significantly lower than that in non anxiety group(P<0.05).Conclusion Anxiety can slow down the recovery rate of patients after radical hysterectomy,make the immune function disorder and reduce the quality of life of patients.Therefore,we should pay more attention to the psychological status of patients and take appropriate psychological intervention measures according to the situation of patients.
