摘　　要：目的:探讨家庭医生签约服务模式对社区糖尿病患者健康管理的效果。方法:选取2018年1月-2019年5月社区糖尿病患者300例,随机分为两组,各150例。对照组实施常规管理;试验组实施家庭医生签约服务管理。比较两组管理效果。结果:管理1年后,试验组血压、空腹血糖、总胆固醇、甘油三酯及低密度脂蛋白胆固醇等指标水平均低于对照组,高密度脂蛋白胆固醇水平高于对照组,差异有统计学意义(P<0.05);试验组糖尿病知识知晓评分及健康管理行为评分均高于对照组,差异有统计学意义(P<0.05)。结论:家庭医生签约服务模式可有效改善糖尿病患者的血压、空腹血糖及血脂水平,并提高健康管理能力。Objective:To explore the effect of family doctor contract service mode on community diabetes health management.Methods:From January 2018 to May 2019,300 cases of diabetic patients in the community were enrolled,they were randomly divided into two groups with 150 cases in each group.The control group carried out routine management.The experimental group carried out the management of family doctor contract service.The effect between two groups was compared.Results:After one year of management,the blood pressure,fasting blood glucose,total cholesterol,blood glucose,total cholesterol,triacylglycerol and low density lipoprotein cholesterol of the experimental group were lower than those of control group,the high density lipoprotein cholesterol level was higher than that in the control group,the differences were statistically significant(P<0.05).The scores of diabetes knowledge and health management behavior in the experimental group were higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:The doctor contract service mode can effectively improve blood pressure,fasting blood glucose and blood lipid level of diabetic patients,and improve their health management ability.
