基于络病理论探讨健脾化瘀法治疗慢性萎缩性胃炎的应用

摘　　要：祖国医学认为,脾胃虚弱为慢性萎缩性胃炎(chronic atrophic gastritis,CAG)发病之根本,瘀阻胃络为其病发的关键,毒损胃络为其转归的重要因素。CAG疾病发生、发展的一系列病理演变与络病理论相符合。该文通过分析络病理论与慢性萎缩性胃炎的相关性,以探讨健脾化瘀法在慢性萎缩性胃炎治疗中的应用。在治疗CAG过程中,健脾化瘀法的功用尤为突出,能防治CAG的发生、发展,减少癌变发生,促进疾病的向愈。健脾多联合益气、渗湿、消食、疏肝、益阴、温肾等法;化瘀多联合通络和胃,同时可酌情辅以行气、活血、破血消积、搜风通络等方法,临床疗效显著。Traditional Chinese medicine believes that the weakness of spleen and stomach is the root of the pathogenesis of chronic atrophic gastritis(CAG),blood stasis blocking the stomach collaterals is the key to its occurrence,and toxic damage to the stomach collaterals is an important factor for its prognosis. A series of pathological changes of CAG disease are consistent with collateral disease theory. This paper analyzes the correlation between collateral disease theory and chronic atrophic gastritis,in order to explore the application of strengthening spleen and replenishing Qi method in the treatment of chronic atrophic gastritis. In the treatment of CAG,the function of invigorating the spleen and removing blood stasis is particularly prominent. It can prevent the occurrence and development of CAG,reduce the occurrence of cancer and promote the recovery of the disease. Strengthening spleen combined with replenishing Qi,excreting dampness,helping digestion,soothing liver,nourishing Yin,warming kidney and other methods;Removing blood stasis mostly combined with promoting Qi and harmonizing stomach,promoting blood circulation,breaking blood and eliminating accumulation,searching wind and unblocking collaterals,etc.,the clinical effect is remarkable.
