益气活血法治疗晚期非小细胞肺癌有效性与安全性的Meta分析

摘　　要：目的系统评价中医益气活血法联合化疗治疗非小细胞肺癌的疗效与安全性。方法计算机检索CNKI、VIP、万方、Pubmed等数据库。由两位研究者独立筛选文献、资料提取和质量评价,检索自建库至2020年1月文献,收集中医药益气活血法联合化疗治疗晚期肺癌随机对照试验(RCT)并按照纳排标准进行筛选。对纳入的文献采用RevMan 5.3软件进行分析和质量评价。结果共纳入符合纳排标准的RCT研究中文文献12篇,1086例患者,Meta分析结果显示:联用益气活血法方药可提高患者化疗有效率[RR=1.39,95%CI(1.22,1.58),n=11,I2=0%,P<0.001],改善患者生存质量评分[MD=9.98,95%CI(7.44,11.07),P<0.001],能够减轻化疗导致的骨髓抑制如白细胞减少[RR=0.47,95%CI(0.33,0.65),n=5,I2=0%,P<0.001],血小板减少[RR=0.52,95%CI(0.35,0.79),n=4,I2=0%,P<0.05]以及减少消化道反应[RR=0.68,95%CI(0.58,0.80),n=8,I2=33%,P<0.001],在肝功能损伤方面没有明显差异(P>0.05)。结论联用益气活血法方药能够提高患者化疗有效率,改善患者生活质量评分,减少骨髓抑制、消化道反应等不良反应的发生,希望更高级别的RCTs进一步进行验证。Objective To systematically evaluate the efficacy and safety of TCM therapy(supplementing Qi and activating blood circulation) combined chemotherapy in the treatment of non-small cell lung cancer(NSCLC).Such databases as PubMed, CNKI,VIP,Wan Fang and CBM were electronically searched to collect the randomized controlled trials(RCT) about TCM therapy combined with chemotherapy compared with single chemotherapy.The data extraction and quality evaluation were independently screened by two researchers.The literature was retrieved from self-built time to July 2019.We appraised the quality of the data and used Rev-Man 5.3 to get the Meta-analysis.Results A total of 12 Chinese RCT studies meeting the inclusion criteria were included, including 1086 patients.The Meta-analysis showed that supplementing Qi and activating blood circulation method can improve the chemotherapy efficiency of patients[RR=1.39,95%CI(1.22,1.58),n=11,I2=0%,P<0.001] and the quality of life[MD=9.98,95%CI(7.44,11.07),I2=0%,P<0.001] and can reduce the myelosuppression caused by chemotherapy, such as leucocyte reduction[RR=0.47,95%CI(0.33,0.65),n=5,P<0.001],thrombocytopenia[RR=0.52,95%CI(0.35,0.79),n=4,I2=0%,P<0.05].It also can reduce gastrointestinal response[RR=0.68,95%CI(0.58,0.80),n=8,I2=33%,P<0.001].It showed no significant difference in liver function injury(P>0.05).Conclusion Supplementing Qi and activating blood circulation method can improve the effective rate of chemotherapy and the quality of life and reduce the occurrence of adverse reactions such as bone marrow suppression and digestive tract reaction.RCTs of a higher level should be required.
