银杏叶片联合盐酸多奈哌齐片治疗血管性认知障碍的疗效及安全性的系统评价和Meta分析

摘　　要：目的系统评价银杏叶片联合盐酸多奈哌齐片治疗血管性认知障碍(vascular cognitive impairment,VCI)的疗效及安全性。方法计算机检索CNKI、万方、VIP、CMB、PubMed、Embase、Cochrane library、Clinical Trials.gov关于银杏叶片联合盐酸多奈哌齐片治疗VCI的随机对照试验。采用“Cochrane偏倚风险评估”工具对所纳入的研究进行质量评价,并运用RevMan5.3软件进行定量Meta分析。结果共纳入11篇文献,总样本量825例。试验组为银杏叶片联合盐酸多奈哌齐片,对照组为盐酸多奈哌齐片。Meta分析结果显示:试验组疗效较好(RR=1.20,95%CI[1.10,1.31],P<0.0001);在改善VCI患者MMSE评分(MD=3.53,95%CI[2.39,4.68],P<0.00001)、MoCA评分(MD=3.17,95%CI[2.59,3.75],P<0.00001)、ADL评分(MD=-3.78,95%CI[-6.72,-0.84],P=0.01)方面,试验组均优于对照组。4项研究报道不良反应,两组不良反应相当(RR=0.83,95%CI[0.44,1.59],P=0.58)。结论银杏叶片联合盐酸多奈哌齐片治疗VCI的疗效优于单用盐酸多奈哌齐片,能有效改善患者认知及提高日常生活能力,且不会增加不良反应。Objective To evaluate the efficacy and safety of Yinxingye Tablet(银杏叶片) combined with donepezil hydrochloride in the treatment of vascular cognitive impairment(VCI).Methods After searching CNKI,Wanfang,VIP,CMB,PubMed,EMBASE,Cochrane Library and Clinical Trials.gov,we collected the randomized controlled studies of Yinxingye Tablet combined with donepezil hydrochloride in the treatment of VCI.And the Revman 5.3 software was used to evaluate the quality and Meta-analysis of the including researches.Results A total of 825 patients with VCI were included in 11 RCTs and the experimental group was treated with Yinxingye Tablet combined with donepezil hydrochloride and the control group was treated with donepezil hydrochloride.Meta-analysis results showed that the effect of the experimental group was better than that of the control group(RR=1.20,95%CI[1.10,1.31],P<0.0001).The experimental group was better than the control group in improving MMSE score(MD=3.53,95%CI[2.39,4.68],P<0.00001),MOCA score(MD=3.17,95%CI[2.59,3.75],P<0.00001)and ADL score(MD=-3.78,95%CI[-6.72,-0.84],P=0.01)of VCI patients.Adverse reactions were reported in four studies,and the two groups had similar adverse reactions(RR=0.83,95%CI[0.44,1.59],P=0.58).Conclusion The efficacy of Yinxingye Tablet combined with donepezil hydrochloride in the treatment of VCI was better than that of donepezil hydrochloride alone,and it could effectively improve patients’ cognition and ability of daily life without increasing adverse reactions.
