赵永烈治疗偏头痛用药规律探讨

摘　　要：目的以赵永烈主任医师门诊偏头痛患者诊疗处方为基础,探索常用药及组方规律。方法收集2017年7月1日—2019年6月30日就诊于北京中医药大学第三附属医院脑病科赵永烈主任医师门诊,符合偏头痛西医及中医诊断标准的偏头痛患者的首诊处方,录入Excel表格中,经数据预处理及转换后,应用SPSS 22.0进行频数频率分析及聚类分析。结果总计79张处方,174种常用药物,累计频次1492次。高频药物(使用频次>10次)47种,应用频次最多的中药为川芎。高频药物种类以解表药、补虚药为主,归经以脾、肺、肝为多,性味以辛温、苦寒之品常见。得出聚类方9首,涉及祛风散寒、疏肝解郁、养血安神、健脾化痰等治法。结论在偏头痛的治疗中,赵师重视川芎的配伍使用及用量,多从肝、脾、肺三脏进行调治,以疏肝、健脾、祛风为大法,内外兼顾,补虚泻实,气血通调,以止头痛。Objective To explore the medication law of migraine by analyzing prescriptions of outpatients in chief physician ZHAO Yonglie′s department.Methods The migraine outpatients′ prescriptions from July 1,2017 to June 30,2019 in professor ZHAO′s Department of The Third Affiliated Hospital of Beijing University of Chinese Medicine were collected.After recording the data in Excel as well as preprocessing and transforming,the methods including frequency analysis and clustering analysis through SPSS 22.0 software were analyzed.Results The study enrolled 79 prescriptions,including 174 drugs.There were 47 drugs with high frequency applications.The frequency analysis also showed the most frequently-used drug was Chuanxiong(Ligusticum Chuanxiong Rhizoma).As for types,they were drugs for relieving the exterior disorder and drugs for tonifying deficiency.Those drugs mainly belonged to the meridians of spleen,lung and liver.The property was warm or cold and the taste was acrid or bitter.Clustering analysis showed there were 9 common prescriptions.The treatment principle involved eliminating the wind,promoting blood flow,smoothing liver,relieving stagnation,nourishing blood,invigorating spleen,dispersing phlegm and so forth.Conclusion It is necessary to attach importance to application and dosage of Chuanxiong(Ligusticum Chuanxiong Rhizoma) in the treatment of migraine.Professor ZHAO prefers to improve the function of liver,spleen and lung,such as smoothing liver,invigorating spleen and dispelling the wind.Finally,the pain could be relieved internally and externally,combining tonification and purgation as well as regulating Qi and blood.
