摘 要:目的比较剪切波弹性成像(SWE)联合超声造影与基于Pl-RADS V2评分的多参数核磁共振(MP-MRI)检查对前列腺结节病灶良恶性的诊断价值。方法回顾性分析2017年12月至2019年12月在本院收治的58例可疑前列腺癌(PCa)患者的临床资料。均行SWE、超声造影、MP-MRI以及术前常规检查。超声引导穿刺活检或手术病理结果作为金标准。对比SWE联合超声造影与MP-MRI诊断PCa的灵敏度、特异度和受试者工作特征曲线的曲线下面积。结果本组58例可疑PCa患者中发现前列腺结节65个。对照穿刺活检的病理显示前列腺癌性结节有32个,前列腺增生结节33个。SWE联合超声造影诊断PCa的灵敏度、特异度分别为75.0%、93.9%。MP-MRI诊断PCa的灵敏度、特异度分别为75.0%、84.8%。SWE联合超声造影和MP-MRI诊断PCa的受试者工作特征(R0C)曲线的曲线下面积分别为0.899、0.830,差异无统计学意义(P=0.170)。结论SWE联合超声造影诊断PCa的效能与MP-MRI相当。Objective To compare the diagnostic value of transrectal shear wave sonoelastog-raphy(SWE)in combination with contrast enhanced ultrasound sonography(CEUS)and multi-parametric MRI(MP-MRI)based on PI-RADS V2 score in benign and malignant prostatic nodules.Methods The clinical data of 58 patients with suspected prostate cancer admitted to our hospital from December 2017 to December 2019 were retrospectively analyzed.All patients underwent SWE,CEUS,MP-MRI and preoperative routine examination.The pathological results of prostate biopsy were the gold standard for diagnosis.The sensitivity,specificity and area under the receiver operating characteristic(ROC)curve of SWE in combination with CEUS and MP-MRI in the diagnosis of prostate cancer were compared.Results Among the 58 cases,65 prostate nodules were suspected as PCa.Biopsy results showed that there were 32 patients with PCa and 33 with BPH.The sensitivity and specificity of SWE combined with CEUS in the diagnosis of prostate cancer were 75.0%and 93.9%,respectively.The sensitivity and specificity of MP-MRI in the diagnosis of prostate cancer were 75.0%and 84.8%,respectively.The area under the ROC curve of the diagnosis of prostate cancer by SWE in combination with CEUS and multi-parameter MRI was 0.899 and 0.830,respectively.There was no statistically significant difference(P=0.170).Conclusions SWE in combination with CEUS is as effective as MP-MRI in the diagnosis of prostate cancer.
