腹腔镜全胃切除联合D2淋巴结清扫术在胃癌患者中的应用

摘　　要：目的探讨腹腔娩全胃切除+D2淋巴清扫治疗胃癌的疗效方法选取2018年1月至2018年12月胃上部癌手术治疗患者40例,按随机投掷法分为对照组(传统开放性手术)与观察组(腹腔铣手术),比较两组手术时间、出血量、淋巴结清扫数量及住院时间、医疗费用、并发症。结果观察组手术时间短于对照组,淋巴结清扫数量多于对照组,且出血量更少,差异有统计学意义(P<0.05);观察组住院时间短于对照组且医疗费用更低,差异有统计学意义(P<0.05);观察组并发症(10.00%)低于对照组(40.00%),差异有统计学意义(P<0.05)结论腹腔镜全胃切除联合D2淋巴结清扫术治疗胃癌患者,创伤小、出血少、术后恢复快,兼具安全性及经济性。Objective To explore the clinical value of laparoscopic total gastrectomy-I-D2 lymphadenectomy in the treatment of gastric cancer.Methods A total of 40 patients with upper gastric cancer who were admitted to Pujiang County People's Hospital from January 2018 to December 2018 were selected and divided into control group(traditional open surgery)and observation group(laparoscopic surgery)according to the random throw method.The time spent on surgery,the amount of bleeding,and the number of lymph node dissections are considered as indicators,and the length of hospitalization,medical expenses,and complications are compared.Results The operation time and the number of lymph node dissections in the observation group were more than those in the control group,and the amount of bleeding was less,the difference was significant(P<0.05);the observation group had shorter hospital stays and lower medical expenses than the control group,the difference was significant(P<0.05).In terms of complications,the observation group(10.00%)was lower than the reference group(40.00%),and the difference was significant(P<0.05).Conclusion Total laparoscopic total gastrectomy combined with D2 lymph node dissection is more valuable in the surgical treatment of gastric cancer patients.It has less trauma and quick recovery after surgery,and is safe and economical.
