基于改良评分体系的彩色多普勒超声结合弹性成像对女性乳腺肿块鉴别诊断的应用

摘　　要：目的评估基于改良评分体系的彩色多普勒超声(CDFI)结合弹性成像诊断乳腺肿块的价值。方法二维超声成像发现乳腺包块后,通过对CDFI以及阻力指数RI、弹性成像获得数据,按照不同权重综合评分,以病理结果为金标准,分别对CDFI、CDFI+RI、弹性成像、综合评分四种方法进行比较,分析乳腺癌诊断的符合率、敏感性以及特异性的差异。结果综合评分法在符合率、敏感性以及特异性等方面优于其他方法敏感性、特异性CDFI最低,仅25%和89.3%,CDH+RI与弹性成像逐渐增高,分别为37.5%、90.0%和43.0%、92.85%。综合评分法(≥6分)最高,分别达到81.25%、95%。约登指数依次为0.143、0.275、0.35、0.76c结论彩色多普勒超声结合弹性成像的改良评分体系,有利于早期发现乳腺肿瘤,无创、可重复,值得临床推广。Objective To explore the value of color Doppler ultrasound combined with elastography in the diagnosis of breast masses based on the improved scoring system.Methods After the breast mass was found by two-dimensional ultrasound imaging,the data were obtained by color Doppler ultrasound(CDFI),resistance index RI,and elastography,and the scores were synthesized according to different weights.The pathological results were used as the gold standard.RI,elastography,and comprehensive scoring were compared to analyze the differences in compliance,sensitivity,and specificity of breast cancer diagnosis.Results The comprehensive scoring method was significantly superior to other methods in terms of coincidence rate,sensitivity,and specificity:the sensitivity and specificity of CDFI were the lowest,only 25%and 89.3%,and CDFI+RI and elastography gradually increased,respectively 37.5%,90.0%and 43.0%,92.85%.The comprehensive scoring method(^6 points)was the highest,reaching 81.25%and 95%respectively.The Jordon index is 0.143,0.275,0.35,and 0.76.Conclusion The improved scoring system of color Doppler ultrasound combined with elastography is helpfi.il for early detection of breast tumors.It is non-invasive and reproducible and can be promoted in clinical practice.
