摘 要:目的探讨β-肌动蛋白(β-actin)对急性脑梗死患者梗死范围的预测价值。方法选取2019年1月至2020年12月在舟山医院接受治疗的210例急性脑梗死患者,根据发病至抽血时间不同分为<6 h组51例,6~12 h组82例,>12 h组77例;选取同期在本院接受治疗的脑出血患者100例和健康体检者50例为对照组。检测并比较各组血清β-actin,分析β-actin预测脑梗死发生以及大面积梗死的效能。结果脑梗死组血清β-actin水平为(1.78±0.30)μg/L,明显高于健康组、脑出血组(均P<0.05);<6 h组和6~12 h患者血清β-actin水平组明显高于>12 h组(均P<0.05)。<6 h组和6~12 h组中,大面积梗死者血清β-actin水平均明显高于中小面积者(均P<0.05);>12 h组中,中小面积与大面积患者血清β-actin水平比较,差异无统计学意义(P>0.05)。发病<6 h、6~12 h时血清β-actin水平预测大面积脑梗死的AUC分别为0.818、0.719。结论急性脑梗死患者血清β-actin表达水平升高,发病12 h内β-actin值与脑梗死面积相关。Objective To investigate clinical value ofβ-actin in predicting infarct size of acute cerebral infarction(ACI).Methods A total of 210 patients with ACI treated in Zhoushan Hospital from January 2019 to December 2020 were enrolled.According to the time from disease onset to blood sampling,the patients were divided into<6 h group(n=51),6-12 h group(n=82)and>12 h group(n=77).Meanwhile,100 patients with cerebral hemorrhage and 50 healthy subjects were selected as control groups.Serumβ-actin was measured and compared between groups.The effect of serumβ-actin on predicting ACI and infarction size was analyzed.Results Serumβ-actin in patients with ACI was(1.78±0.30)μg/L,which was significantly higher than that in healthy subjects and patients with cerebral hemorrhage(all P<0.05).The serumβ-actin in<6 h and 6-12 h groups were significantly higher than that in>12 h group(all P<0.05).In<6 h and 6-12 h groups,serumβ-actin was significantly higher in patients with large infarction volume than with moderate/smaller infarction volume(all P<0.05).Serumβ-actin was not significantly different between patients with larger and smaller/moderate infarction volume in>12 h group(P>0.05).The area under ROC curve of serumβ-actin in predicting massive cerebral infarction at disease onset<6 h and 6-12 h was 0.818 and 0.719,respectively.Conclusion The expression of serumβ-actin is increased in patients with ACI.The value of serumβ-actin within 12 hours after disease onset is related to cerebral infarction size.
