摘 要:目的探讨四次跨膜蛋白CD81在胶质瘤组织中的表达水平,分析其与预后的关系。方法收集自2011年6月至2014年6月雅安市人民医院手术切除的114例脑胶质瘤组织纳入胶质瘤组,另选取同期48例脑外伤后行颅脑损伤内减压术中切取的正常脑组织纳入正常组。随机选取不同级别胶质瘤组织与正常脑组织各12例,应用逆转录-聚合酶链反应和蛋白免疫印迹法检测CD81基因和蛋白的表达水平。应用免疫组织化学染色法检测所有组织中CD81的表达水平,根据染色结果分为高表达和低表达。分析CD81表达水平与患者总体生存时间的关系。结果胶质瘤组织中CD81 mRNA相对表达量明显高于正常脑组织(P<0.05),且随着胶质瘤按世界卫生组织(WHO)分级的增高,CD81 mRNA相对表达水平逐渐增高(P<0.05)。胶质瘤组织中CD81蛋白相对表达量明显高于正常脑组织(P<0.05),且随着胶质瘤WHO分级的增大,CD81蛋白相对表达水平逐渐增高(P<0.05)。免疫组织化学检测结果显示,正常脑组织中CD81少量表达,而胶质瘤组织中CD81阳性表达较高。正常脑组织中CD81高表达率为18.8%(9/48)、WHOⅡ级胶质瘤组织CD81高表达率为25.0%(9/36)、WHOⅢ级组织为44.7%(21/47)、WHOⅣ级组织为74.2%(23/31)。COX因素分析结果显示,术前卡氏评分<80分、WHO分级为Ⅲ~Ⅳ级、CD81高表达是胶质瘤患者总体生存时间较低的独立影响因素(P<0.05)。CD81低表达和高表达胶质瘤患者的中位生存时间分别为18个月和11个月。CD81高表达患者的总体存活率明显低于低表达患者,差异有统计学意义(P<0.05)。结论胶质瘤组织中CD81基因和蛋白的表达水平明显高于正常脑组织,CD81表达水平增高可预示患者的不良生存预后。Objective To investigate the expression of CD81 in glioma tissue and its relationship with prognosis.Methods A total of 114 cases of brain glioma tissues surgically resected in Ya′an People′s Hospital from June 2011 to June 2014 were included in the glioma group,and 48 cases of normal brain tissues removed during intracerebral decompression after traumatic brain injury during the same period were included in the normal group.Glioma tissues of different grades and normal brain tissues were randomly selected in 12 cases.The expression levels of CD81 gene and protein were detected by reverse transcription-polymerase chain reaction and western blot.Immunohistochemical staining was used to detect the expression of CD81 in all tissues.According to the staining results,CD81 expression was divided into high expression and low expression.The relationship between CD81 expression level and overall survival time of patients was analyzed.Results The relative expression level of CD81 mRNA in glioma tissues was significantly higher than that in normal brain tissues(P<0.05),and the relative expression level of CD81 mRNA was gradually increased with the increase of glioma classification according to the World Health Organization(WHO),with statistical significance(P<0.05).The relative expression level of CD81 protein in glioma tissue was significantly higher than that in normal brain tissue(P<0.05),and the relative expression level of CD81 protein was gradually increased with the increase of glioma WHO grade(P<0.05).Immunohistochemical results showed that CD81 was slightly expressed in normal brain tissues,while the positive expression of CD81 was higher in glioma tissues.The high expression rate of CD81 was 18.8%(9/48)in normal brain tissue,25.0%(9/36)in WHOⅡgrade glioma tissue,44.7%(21/47)in WHOⅢgrade glioma tissue,and 74.2%(23/31)in WHOⅣgrade glioma tissue.Cox factor analysis results showed that preoperative Karnofsky score<80,WHO gradeⅢ~Ⅳand high expression of CD81 were independent influencing factors for lower o
