摘　　要：目的:分析不植骨前提下上颌后牙区骨量不足患者上颌窦内外提升术的近远期效果。方法:纳入96例(种植体126颗)上颌后牙区骨量不足患者,依照入院顺序分组,A组患者48例接受上颌窦内提升术,B组48例接受上颌窦外提升术治疗;两组均在不植骨的前提下进行手术,对比两组治疗近(1年)远(5年)期效果。结果:术后24 h随访1年两组患者剩余牙槽嵴高度、平均骨增量对比,A组均高于B组(P<0.05)。随访5年A组患者平均骨增量高于B组(P<0.05)。结论:上颌后牙区骨量不足患者,在不考虑植骨前提下,采取上颌窦内或外提升术治疗,近远期种植体稳定性良好,种植成功率高。Objective:To analyze the short-term and long-term results of maxillary sinus lift surgery without bone grafting for implantation in patients with insufficient bone mass in the posterior maxillary area.Methods:96 patients(126 implants)with insufficient bone mass in the posterior maxillary area were divided into 2 groups(n=48)according to the order of admission.The patients in group A underwent maxillary sinus internal lift,and 48 in group B underwent external maxillary sinus lift surgery;the subjects in both groups were operated without bone grafting,the patients were followed-up for 5 years,the 1 year-and 5 year-effects of the 2 groups were compared.Results:At 1 year follow up the remaining alveolar ridge height and average bone increased in the 2 groups.The increase of group A was higher than that of group B(P<0.05).At the 5-year follow-up,the average bone gain of group A was higher than that of group B(P<0.05).Conclusion:For patients with insufficient bone mass in the posterior maxillary area,internal and external elevation of the maxillary sinus without bone grafting are effective for implantation.The implants are stable in the short and long term,and the implantation success rate is high.
