尿液microRNA-21表达联合三维超声成像技术对膀胱尿路上皮癌的诊断价值

摘　　要：目的探讨尿液中micro RNA-21(miR-21)表达联合三维超声成像技术(3D-US)在膀胱尿路上皮癌(BUC)诊断中的临床应用价值。方法收集78例BUC患者(病例组)和80例良性泌尿系疾病患者(对照组)的尿液,应用实时荧光定量PCR(q PCR)检测尿液中miR-21的表达水平;分析miR-21水平与临床病理学特征的关系;行3D-US检查,并采用受试者工作特征(ROC)曲线分析尿液miR-21及miR-21联合3D-US对BUC诊断的临床价值。结果病例组患者尿液miR-21的相对表达量显著高于对照组(P<0.01)。尿液miR-21在浸润组中含量明显高于非浸润组,在高级别组中表达明显高于低级别组(P<0.05);ROC曲线分析显示:尿液miR-21表达联合3D-US诊断BUC的敏感度为94.87%,特异度为96.25%,约登指数接近于膀胱镜检查。结论 BUC患者尿液中miR-21表达上调可能与肿瘤进展有关。尿液miR-21联合3D-US可作为筛查BUC的一项有效且无创的诊断方法。Objective To investigate the clinical value of microRNA-21(miR-21)expression in urine combined with three-dimensional ultrasound imaging(3D-US)in the diagnosis of urinary urothelial carcinoma(BUG).Methods The urine samples of 78 BUC patients(the case group)and 80 patients with benign urinary tract diseases(the control group)were collected.The expression level of miR-21 in urine was detected by real-time quantitative PCR(qPCR).The relationship between miR-21 level and clinicopathological characteristics was analyzed.The 3D-US examination was performed,and receiver operating characteristic(ROC)curve was used to analyze the clinical value of urine miR-21 and miR-21 combined with 3D-US for BUC diagnosis.Results The relative expression level of miR-21 in urine was significantly higher in case group than that of control group(P<0.01).The content of urine miR-21 was significantly higher in the infiltrating group than that in the non-infiltrating group,and the expression of urine miR-21 was significantly higher in the high-grade group than that in the low-grade group(P<0.05).ROC curve analysis showed that the sensitivity and specificity of urine miR-21 expression combined with 3D-US were 94.87%and 96.25%in the diagnosis of urothelial carcinoma,and Youden index was close to the result of cystoscopy.Conclusion The upregulated expression of miR-21 in urine of BUC patients may be related to tumor progression.Urine miR-21 combined with 3D-US can be used as an effective and non-invasive diagnostic method for screening BUC.
