摘 要:目的探讨十伤灵促进难愈性创面愈合的机制,以期为临床治疗和研究提供依据。方法采用随机数字表法将36只SPF级健康雄性Wistar大鼠随机分为生理盐水组、贝复济组、十伤灵组,每组12只。3组均采用外科手术联合腹部注射氢化可的松琥珀酸钠的方法制备难愈性溃疡模型,术后十伤灵组和贝复济组分别用生理盐水和十伤灵、贝复济行常规换药处理,生理盐水组行常规生理盐水冲洗,均2次/d,连续14 d。分别于造模7 d、14 d后通过数码相机结合Image J软件计算各组的溃疡愈合率,采用免疫组化法检测新鲜肉芽组织中转化生长因子β1(TGF-β1)、血小板衍生因子(PDGF)表达情况。结果各组大鼠造模14 d后的创面愈合率均明显高于造模7 d后(P均<0.05),且十伤灵组创面愈合率明显高于贝复济组和生理盐水组(P均<0.05)。造模7 d后,十伤灵组大鼠创面组织中TGF-β1和PDGF表达量均明显高于生理盐水组(P均<0.05),且十伤灵组创面组织中PDGF表达量明显高于贝复济组(P<0.05)。造模14 d后,十伤灵组大鼠创面中TGF-β1和PDGF表达量均明显低于生理盐水组及贝复济组(P均<0.05)。结论十伤灵能提高大鼠难愈性创面的愈合率;肉芽组织生长期十伤灵可通过增加TGF-β1和PDGF的分泌,促进成纤维细胞和毛细血管生长,促进创面愈合;成熟期十伤灵可减少TGF-β1和PDGF的合成,防止创面过度增生。Objective It is to explore the mechanism of Shishangling in promoting the healing of refractory wounds in order to provide some reference for clinical treatment and research.Methods Thirty-six SPF-grade healthy male Wistar rats were randomly divided into normal saline group,Beifuji group,and Shishangling group,with 12 rats in each group by the random number table method.The three groups all used surgical operation combined with abdominal injection of hydrocortisone sodium succinate to prepare refractory ulcer models.After operation,the Shishangling group and the Beifuji group were treated with normal saline plus Shishangling and Beifuji for routine dressing change,and the normal saline group was treated with normal saline for dressing change,2 times per day for 14 consecutive days.The ulcer healing rate of each group was calculated by digital camera combined with ImageJ software on 7 d and 14 d after modeling,and the expression of transforming growth factorβ1(TGF-β1)and platelet-derived growth factor(PDGF)in fresh granulation tissue were detected by immunohistochemistry.Results The wound healing rate of rats in each group 14 days after modeling was significantly higher than that 7 days after modeling(all P<0.05),and the wound healing rate of Shishangling group was significantly higher than that of Beifuji group and normal saline group(P both<0.05).7 days after modeling,the expressions of TGF-β1 and PDGF in the wound tissue of the Shishangling group were significantly higher than those in the normal saline group(both P<0.05),and the expression of PDGF in the wound tissues of the Shishangling group was significantly higher than that of the Beifuji group(P<0.05).14 days after modeling,the expressions of TGF-β1 and PDGF in the wounds of the Shishangling group were significantly lower than those of the normal saline group and the Beifuji group(all P<0.05).Conclusion Shishangling can improve the healing rate of intractable wounds in rats;Shishangling can increase the secretion of TGF-β1 and PDGF to promote the gro
