摘  要:目的探讨葡萄糖调节蛋白(GRP78)和碱性成纤维细胞生长因子(bFGF)在人喉鳞癌组织中的表达及与微血管生成的关系。方法收集2015年1月—2019年6月于自贡市第四人民医院耳鼻咽喉头颈外科收治住院手术的60例喉癌患者术后标本,应用免疫组化SP法分别检测60例喉鳞癌组织标本和32例癌旁组织标本中GRP78和bFGF表达情况,并用D2-40阳性产物定位淋巴管内皮细胞胞浆及细胞膜,计数微血管密度(MVD),分析GRP78、bFGF、MVD与临床病理因素的关系和GRP78、bFGF、MVD之间的关系。结果喉鳞癌组织中GRP78和bFGF阳性表达率分别为78.33%(47/60)和83.33%(50/60),癌旁组织中分别为37.50%(12/32)和34.38%(11/32),癌组织中阳性表达率均明显高于癌旁组织(P均<0.05);高倍视野下,喉鳞癌组织中MVD值为25.12±3.77,癌旁组织MVD值为14.65±3.80,喉鳞癌组织中MVD值明显高于癌旁组织,差异有统计学意义(P<0.05)。GRP78和bFGF的阳性表达与喉鳞癌的分化程度、有无淋巴结转移及临床分期密切相关(P均<0.05),与有无吸烟史、肿瘤大小、性别及年龄无相关性(P均>0.05);MVD值与淋巴结转移显著相关(P<0.05),与分化程度、临床分期、肿瘤大小、有无吸烟病史、性别及年龄均无相关性(P均>0.05)。GRP78和bFGF在喉鳞癌组织中表达呈显著正相关(P<0.05)。喉鳞癌组织中GRP78和bFGF阳性表达者的MVC值均明显高于二者阴性表达者(P均<0.05)。结论GRP78和bFGF在喉鳞癌的发展中起重要作用,二者可作为喉鳞癌转移及预后的预判指标,通过抑制GRP78和bFGF的表达可减少微血管生成,从而抑制癌细胞生长。Objective It is to investigate the expression of glucose regulatory protein(GRP78)and basic fibroblast growth factor(bFGF)in human laryngeal squamous cell carcinoma and their relationship with microangiogenesis.Methods Postoperative specimens of 60 patients with laryngeal cancer who were admitted to the Department of Otorhinolaryngology Head and Neck Surgery of Zigong Fourth People’s Hospital from January 2015 to June 2019 were collected after surgery.The expression of GRP78 and bFGF in 60 specimens of laryngeal squamous cell carcinoma and 32 specimens of adjacent tissues were detected by immunohistochemical SP method,and D2-40 positive product was used to locate lymphatic endothelial cell cytoplasm and cell membrane,the microvessel density(MVD)was counted,the relationship between GRP78 and bFGF expression and MVD and clinicopathological factors,and the correlation between GRP78 and bFGF and MVD were analyzed.Results The positive expression rates of GRP78 and bFGF in laryngeal squamous cell carcinoma tissues were 78.33%(47/60)and 83.33%(50/60),respectively,and those in paracancerous tissues were 37.50%(12/32)and 34.38%(11/32),respectively,the positive expression rate in cancer tissues was significantly higher than that in adjacent tissues(all P<0.05);under high power field,MVD value in laryngeal squamous cell carcinoma tissue was 25.12±3.77,MVD value in adjacent tissues was 14.65±3.80,the MVD value in laryngeal squamous cell carcinoma tissue was significantly higher than that in adjacent tissues,and the difference was statistically significant(P<0.05).The positive expression of GRP78 and bFGF was closely related to the degree of differentiation of laryngeal squamous cell carcinoma,the presence or absence of lymph node metastasis,and the clinical stage(all P<0.05),and was not correlated with the presence or absence of smoking history,tumor size,gender,and age(all P>0.05);MVD value was significantly correlated with lymph node metastasis(P<0.05),but had no correlation with differentiation,clinical stage,tumor si
