免疫球蛋白检验对乙型肝炎患者的临床效果观察

摘　　要：目的探讨乙型肝炎患者选用免疫球蛋白检验的价值。方法纳入110例乙型肝炎患者研究(2018年5月12日—2020年5月12日),按病情严重程度分为轻度组(n=40)、中度组(n=40)、重度组(n=30),同期选择40例健康体检者作为对照组,采集150例受检者空腹静脉血,使用免疫散射比浊法测定免疫球蛋白水平,分析免疫球蛋白水平。结果(1)不同人群:乙型肝炎者免疫球蛋白A(immunoglobulin A,IgA)、免疫球蛋白G(immunoglobulin G,IgG)、免疫球蛋白M(immunoglobulin G,IgM)高于对照组,其中轻度<中度<重度,P<0.05。(2)疗效:病情缓解组IgA(2.61±0.15)g/L、IgG(14.26±4.21)g/L、IgM(2.61±0.15)g/L、总胆红素(total bilirubin,TBIL)(167.82±103.18)μmol/L低于病情未缓解组,凝血酶原时间活动度(prothrombin time activity,PTA)(52.88±15.74)%高于病情未缓解组,t=18.2389、3.0754、23.4536、7.9458、9.2646,P<0.05。结论免疫球蛋白检验在乙型肝炎病情及预后效果评估中极具优势,可为临床治疗该疾病提供参考。Objective To explore the value of immunoglobulin test for patients with hepatitis B.Methods A study of 110 patients with hepatitis B(from May 12,2018 to May 12,2020)was included.According to the severity of the disease,they were divided into mild group(n=40),moderate group(n=40),and severe group(n=30).40 healthy subjects served as a control group.Fasting venous blood was collected from 150 subjects,and immunoglobulin levels were determined by immunoscattering turbidimetric method.The immunoglobulin levels were analyzed.Results(1)Different populations:immunoglobulin A(IgA),immunoglobulin G(IgG),immunoglobulin M(IgM)of hepatitis B patients were higher than those of the control group,among which mild<moderate<severe,and P<0.05.(2)Efficacy:IgA(2.61±0.15)g/L,IgG(14.26±4.21)g/L,IgM(2.61±0.15)g/L,total bilirubin(TBIL)(167.82±103.18)μmol/L in the remission group were lower than the non-remission group.The prothrombin time activity(PTA)(52.88±15.74)%of the remission group was higher than that of the non-remission group,and t=18.2389,3.0754,23.4536,7.9458,9.2646,all P<0.05.Conclusion Immunoglobulin test has great advantages in evaluating the condition and prognosis of hepatitis B.It can provide reference for clinical treatment of the disease.
