乳腺良性肿瘤切除术切口的选择及应用

摘　　要：目的探讨各个类型的乳腺良性肿瘤切除手术切口的选择以及优势,以方便为临床的相关治疗工作提供科学的参考。方法选择来我院进行治疗的500例乳腺良性肿块患者作为调查对象,时间为2018年3月至2019年3月,对其临床资料进行总结和回顾性分析。在治疗过程中,根据患者的实际情况为患者选择对应的手术切口,并比较各个类型手术切口的治疗效果和美观效果。结果所有患者均顺利完成乳腺肿块切除手术,均不存在局部残留和复发。手术切口1期愈合494例患者,5例患者存在脂肪坏死,延期愈合,其中3例患者为腋窝切口,2例转为传统放射切口,有1例患者合并缝线反应,对愈合效果产生影响;所有病例随访至2019年9月,传统的放射状切口和乳房下皱襞切口存在明显瘢痕,而环乳晕切口、腋窝切口、纵乳晕切口均无明显瘢痕。结论临床对乳腺良性肿瘤切除的患者进行乳晕区域的切口手术可在一定程度上保证乳腺良性肿瘤切口的安全性和美观性。Objective To explore the selection and advantages of surgical incisions for various types of benign breast tumors,so as to provide scientific reference for clinical related treatments.Methods A total of 500 patients with benign breast masses who came to our hospital for treatment from March 2018 to March 2019 were selected as the survey subjects,and the clinical data of all patients were summarized and retrospectively analyzed.In the treatment process,the corresponding surgical incisions were selected for the patients according to the actual conditions of the patients,and the therapeutic effects and aesthetic effects of various types of surgical incisions were compared.Results All patients successfully completed the resection of breast masses,and there was no local residual or recurrence.Surgical incisions were first healed in 494 patients,and 5 patients had fat necrosis and delayed healing.Among them,3 patients had axillary incisions,2 were converted to traditional radiation incisions,and 1 patient had suture reaction,which had an impact on the healing effect.All cases were followed up to September 2019.There were obvious scars in the traditional radial incisions and subbreast fold incisions,but there were no obvious scars in the circular areola incisions,axillary incisions,and longitudinal areola incisions.Conclusion Clinical incision surgery in the areola area for patients with benign breast tumors can ensure the safety and aesthetics of the incisions for benign breast tumors to a certain extent.
