CT扫描与X-线钡餐检查诊断胃癌的价值比较

摘　　要：目的:比较CT与X-线钡餐造影检查对胃癌的诊断价值。方法:回顾性分析2017年7月—2020年10月我院收治的91例胃癌患者的临床资料,以术后病理结果为金标准,比较CT检查与X-线钡餐造影检查对胃癌的临床诊断价值。结果:CT检查确诊率96.70%显著高于X-线钡餐造影检查结果85.71%(P<0.05),两种方法的漏诊率(3.30%,9.89%)及误诊率(0.00%,4.40%)比较差异无统计学意义(P>0.05);ROC曲线下,CT检查的AUC面积0.896、灵敏度95.68%、特异度100.00%、约登指数0.746均高显著于X-线钡餐造影检查结果(AUC面积0.768、灵敏度85.33%、特异度95.60%、约登指数0.557),差异有统计学意义(P<0.05)。结论:两种方法均是诊断胃癌的有效手段,相较于X-线钡餐造影,CT检查对胃癌的诊断准确率及诊断价值更高,更值得临床应用推广。Objective To compare the diagnostic value of CT and X-ray barium meal in gastric cancer.Methods The clinical data of 62 patients with gastric cancer from July 2017 to October 2020 were retrospectively analyzed.The clinical diagnostic value of CT examination and X-ray barium meal examination was compared based on the postoperative pathological results as the gold standard.Results 60 cases were confirmed by CT examination,and the diagnosis rate 96.70%was significantly higher than that of X-ray barium meal examination rate 85.71%(P<0.05).There was no significant difference in the missed diagnosis rate(3.30%,9.89%)and misdiagnosis rate(0.00%,4.40%)between the two methods(P>0.05).Under ROC curve,AUC area,sensitivity,specificity and Youden index of CT examination were 0.896,95.68%,100.00%and 0.746,respectively,which were significantly higher than those of X-ray barium meal examination.The results of contrast examination(AUC area 0.768,sensitivity 85.33%,specificity 95.60%,Youden index 0.557),the difference was statistically significant(P<0.05).Conclusion Both methods are effective in the diagnosis of gastric cancer.Compared with X-ray barium meal,CT examination has higher diagnostic accuracy and diagnostic value for gastric cancer.
