摘　　要：目的:探讨接纳与承诺疗法对创伤性截肢患者反刍性沉思和生活质量的影响。方法:选取2018年11月-2020年12月在某院手足显微外科住院的72例创伤性截肢患者,采用随机数字表法分为观察组和对照组,每组36例,对照组给予常规健康教育,观察组在对照组基础上实施接纳与承诺疗法干预,比较干预前后两组患者的反刍性沉思和生活质量评分情况。结果:最终68例完成研究,干预组和对照组各34例。干预后,观察组患者反刍性沉思评分及其两个维度侵入性反刍性沉思、目的性反刍性沉思评分均显著优于对照组,差异具有统计学意义(t=-3.101,4.472,-7.954;P<0.05);干预后,观察组患者的生活质量中PCS、MCS、TS评分均优于对照组,差异具有统计学意义(t=10.199,13.668,13.314;P<0.05)。结论:接纳与承诺疗法可提高创伤性截肢患者的反刍性沉思水平,改善其生活质量。Objective:To investigate the effects of admission and commitment therapy on ruminant meditation and quality of life in patients with traumatic amputation.Methods:In November 2018 to December 2020 in our hospital the brotherhood of microsurgical hospital,72 cases of patients with traumatic amputation were selected using the random number table method,which were divided into observation group and control group,36 cases in each group.Control group was given routine health education,observation group,on the basis of the control group was given acceptance and commitment therapy intervention.After the intervention,ruminant meditation and quality of life score were compared between two groups of patients.Results:A total of 68 cases completed the study,34 cases in the intervention group and 34 cases in the control group.After intervention,the scores of ruminant meditation,invasive ruminant meditation,and purposive ruminant meditation in the observation group were significantly lower than those in the control group(t=-3.101,4.472,-7.954,P<0.05);After intervention,the scores of PCS,MCS and TS in the observation group were all statistically significantly higher than those in the control group(t=10.199,13.668,13.314,P<0.05).Conclusion:Acceptance and commitment therapy can improve ruminant meditation and quality of life in patients with traumatic amputation.
